摘要
目的探讨腰椎转移瘤磁共振(MRI)表现,为其诊断提供参考。方法对56例腰椎转移瘤患者进行MRI检查,并对MRI下病灶部位、形态、信号强度进行回顾性分析。结果 56例患者受累椎体共计130个,并且以多个椎体受累为主;侵及椎体附件32例(57.1%),侵及椎旁及椎管内软组织25例(44.6%);受累椎体呈楔形改变38例(67.9%),10例(17.9%)呈膨胀性改变;溶骨性转移39例(69.6%),成骨性转移5例(8.9%),混合性转移12例(21.4%)。溶骨性转移瘤以低T1WI、高T2WI、低STIR为主,成骨性以低T1WI、低T2WI、低STIR为主,混合性转移瘤中以混合信号为主,STIR信号以高信号为主。结论 MRI检查可显示腰椎转移瘤的形态、位置、侵袭程度及信号强度,对腰椎转移瘤的诊断及治疗具有指导意义。
Objective To explore the magnetic resonance（MRI） manifestation of metastatic lumbar vertebral tumor. Methods Fifty six patients with metastatic lumbar vertebral tumor received MRI examination. The location and shape of the focus and the signal intensity under MRI were retrospectively analyzed. Results There were 130 involved vertebrae which were mainly multiple ones among 56 patients. The appendixes of vertebrae of 32 cases （ 57.1% ） were invaded, and the soft tissues near and in the vertebrae of 25 ones （44.6%）were invaded. The involved vertebrae had the changes of wedge shape in 38 cases（67.9% ）and distensibility in 10 ones （ 17.9% ）. Osteolytic metastasis occurred in 39 cases （69.6%） ; ossification metastasis occurred in 5 ones （ 8.9% ） ; mixed metastasis occurred in 12 ones （21.4% ）. The osteolytic metastasis showed mainly low T1WI, high T2 WI, and low STIR;the ossification metastasis showed mainly low T1WI, low T2WI, and low STIR;the mixed metastasis showed mainly the mixed signal, and the STIR signal was consisted mainly of the high signal. Conclusion MRI detection can show the shape, location, the invasive degree, and the signal density of metastatic lumbar vertebral tumor. It plays a great role in the diagnosis and treatment of metastatic lumbar vertebral tumor.
