超短波疗法结合运动康复训练治疗脑卒中偏瘫患者Ⅰ期肩手综合征的效果观察 被引量:7

The effect of ultrashort wave therapy and movement rehabilitation training on hemiplegic stroke patients with stage Ⅰ shoulder-hand syndrome

摘要 目的 观察超短波物理疗法结合运动康复训练治疗脑卒中偏瘫患者Ⅰ期肩手综合征的临床疗效.方法 将首都医科大学教学医院2010年9月~2012年9月60例患有Ⅰ期肩手综合征的脑卒中偏瘫患者随机分为治疗组(n=30)和对照组(n=30),对照组给予常规康复训练,治疗组在常规运动康复训练同时给予超短波物理疗法,每日1次,共4周.两组在治疗前后分别进行简式Fugl-Meyer运动功能评分、日常生活活动能力评分、疼痛-目测比评分(VAS)、水肿分级评分,观察治疗后两组评分变化,并根据分数评定判断疗效.结果 治疗后4周,治疗组患者上肢及手的运动功能Fugl-Meyer评分及改良Barthel指数分别为(25.36±4.11)分及(74.51±13.03)分,较对照组明显增高(t=3.062,P=0.007;t=2.314,P=0.033),上肢自发性疼痛VAS评分和水肿评分分别为(3.08±1.05)分与(2.81±1.03)分,改善程度明显优于对照组(t=2.138,P=0.041;t=2.060,P=0.044).结论 超短波物理疗法结合运动康复训练能进一步缓解Ⅰ期肩手综合征的脑卒中偏瘫患者上肢水肿及自发性疼痛程度,提高患侧上肢运动功能. Objective To observe the clinical effect of ultrashort wave therapy combine movement rehabilitation train- ing on hemiplegic stroke patients with stage I shoulder- hand syndrome. Methods 60 hemiplegic stroke patients with stage I shoulder- hand syndrome in Teaching Hospital of Capital Medical University from September 2010 to September 2012 were randomly divided into the treatment group (n=30) and control group (n=30). The control group received routine movement rehabilitation training, and the treatment group received conventional rehabilitation training and ultrashort wave physical therapy, 1 time a day, for 4 weeks. The two groups were respectively evaluated by Fugl- Meyer motor function score, the activities of daily living score, pain visual analogue score (VAS), edema score before and after treatment, and the curative effect according to the scores were evaluated. Results 4 weeks after treatment, the Fugl-Meyer score and modified Barthel index in upper extremity and hand of treatment group patients were (25.36± 4.11) scores and (74.51±13.03) scores, which were significantly higher than that of the control group (t = 3.062, P = 0.007; t = 2.314, P = 0.033). The spontaneous pain VAS score and edema scores in upper limb of treatment group were (3.08±1.05) scores and (2.81±1.03) scores, which were significantly improved than that of control group (t = 2.138, P = 0.041; t = 2.060, P = 0.044). Conclusion Ultrashort wave therapy combine movement rehabilitation train- ing can further reduce the edema and spontaneous pain, improve the motor function of upper limbs in hemiplegic stroke patients with stage I shoulder- hand syndrome.
