摘要
目的探讨卵巢非肿瘤性囊性病变MRI的影像特点。方法回顾性分析182例有完整临床资料的卵巢非肿瘤性囊性病变的MRI影像,其中平扫60例,平扫+增强122例。观察病灶的部位、形态、大小、分房、囊液信号、囊壁或分隔厚度等,总结其影像特点。结果卵巢非肿瘤性囊性病变可分为功能性和非功能性囊肿。功能性囊肿分为卵巢囊肿(15例)、成熟卵泡(25例)、黄体囊肿(25例)、黄素囊肿(20例)、出血性囊肿(15例);非功能性囊肿分为巧克力囊肿(32例)、多囊卵巢综合征(19例)、卵巢输卵管脓肿(25例)、输卵管积水伴卵巢炎(6例)。根据囊液中是否有出血及出血的不同时期,MR表现多样:可呈长T1等T1或短T1信号,T2WI可呈等-稍高-高信号;Gd-DTPA增强后扫描,功能性囊肿动脉期囊壁轻微强化,静脉及延迟期中等度强化,其强化程度超过卵巢间质信号;非功能性囊肿及卵巢非肿瘤性囊性病变囊壁较厚且有明显延迟强化的特征;所有病变中囊液均无强化。结论卵巢非肿瘤性囊性病变MRI平扫时囊液T1、T2信号变化多样,增强扫描时囊液无强化,囊壁光整且有不同程度的强化和延迟强化。
Objective To investigate MRI features in ovarian nonneoplastic cystic lesions. Methods A total of 182 patients with ovarian nonneoplastic cystic lesions proven by surgery and pathology underwent MRI （non-enhanced scan in 60 cases, non-en- hanced scan plus enhanced scan in 122 cases）. Their MRI features, including location, shape, size, fluid signal intensity, and thickness of septa and wall, were evaluated and compared. Results Ovarian nonneoplastic cystic lesions were divided into func- tional cysts and nonfunctional cysts. The functional cysts included ovarian cyst （n = 15 ）, follicular cyst （n = 25 ）, corpus luteum cyst （ n = 20 ） and hemorrhagic cyst （ n = 15 ） ; nonfunctional cysts consisted of chocolate cyst （ n = 32 ）, polycystic ovary disease （n = 19） , adnexal abscess （n = 25 ） and hydrosalpinx （n = 6 ）. There were a great many of MRI findings, including high, equal or low signal on T1WI and equal, slightly high or high signal on T2WI, according to the bleeding and its different phase in the cysts. On enhanced scan, functional cysts＇ walls showed gentle enhancement in the arterial phase and continuous or progressive en- hancement in the venous and delaying phase, whose degree exceeded the ovarian stroma. On enhanced scan, the cystic walls owned progressive enhancement speciality in the nonfunctional cysts and nonneoplastic lesions. Cystic liquid was of no enhancement in these cases. Conclusion MRI is accurate for demonstrating morphological features of ovarian nonneoplastic cystic lesions. In plain scanning, the signal of cystic liquid varies in T1WI and T2 WL In contrast scanning, the cystic liquid is not concentrated, but cystic walls shows enhancement and delayed enhancement.
