胎盘植入的超声表现分析 被引量：17

摘要 目的:探讨胎盘植入产前彩超的声像图特征。方法:回顾性分析经病理证实的13例胎盘植入的超声图像,其中产前超声检出5例,漏诊8例。结果:胎盘植入声像图特征:胎盘内多发血池,胎盘后间隙消失或不规则,部分区域子宫肌层缺失,甚至完全中断。剖宫产子宫胎盘低置并穿透性胎盘,前壁下段浆膜层与膀胱后壁分界不清,胎盘穿入膀胱内者膀胱内壁毛糙甚者胎盘回声可向膀胱内突。彩色多普勒未见正常宫壁静脉丛,而代之以迂曲增宽的血管,胎盘似置于"火海样"血管之上。结论:彩超孕期追踪检查胎盘植入情况尤其是穿透性胎盘具有重要价值,可提示继续妊娠有子宫自发破裂的风险,同时为临床选择治疗方案提供可靠依据。
