摘要
目的：探讨彩色多普勒超声对胎盘植入尤为穿透性胎盘的诊断要点及分析.方法：本组对手术病理证实的18例胎盘植入患者二维声像图及彩色多普勒特征进行回顾分析.结果：18例患者中仅有3例是穿透性胎盘超声诊断或疑为胎盘植入,诊断率仅为16%.穿透性胎盘植入超声特点为胎盘完全覆盖宫颈内口,面积增大增厚,胎盘内多发血池,胎盘后间隙消失,肌壁变薄显示不清,CDFI未见正常宫壁静脉丛,而代之以迂曲增宽的血管,胎盘似置于＂火海样＂血管之上.结论：彩色多普勒超声在产前产后检查胎盘有无植入具有重要应用价值,为临床选择制定合理的治疗方案提供可靠的诊断依据,指导临床及时采取相应的手术措施.
Objective： Discusses colored Doppler supersonic to implant the especially penetrability placenta to the placenta the diagnosis main point and the analysis. Methods：This group 18 example placentas which confirmed to the surgery pathology implants the patient two-dimensional acoustic image chart and the colored Doppler characteristic carries on the review analysis. Results： In 18 example patients only has 3 examples is the penetrability placenta ultrasound diagnosis or doubts for the placenta implants, the diagnosis rate only is 16 %. The penetrability placenta implants the supersonic characteristic to cover cervix of the uterus boring completely for the placenta, the area increases the accumulation,in the placenta sends the blood pond, after the placenta gap vanishing, the myowall changes the thin demonstration not to be unclear,CDFI has not seen the normal palace wall vein clump, but the generation by in and out increases the width the blood vessel, the placenta resembles puts in ＂the sea of fire type＂ above the blood vessel. Conclusion： Whether there is does colored Doppler supersonic implant in the pre-natal post-natal inspection placenta has the important application value,is the clinical choice formulation reasonable treatment plan provides the reliable diagnosis basis, the instruction clinical takes the corresponding surgery measure promptly.
