摘要
目的探讨胸腔注射尿激酶联合微波理疗治疗结核性包裹性胸腔积液的疗效。方法将2007年6月—2012年5月在本院呼吸科住院治疗结核性包裹性胸腔积液的60例患者随机分为对照组、尿激酶治疗组和尿激酶联合微波治疗组,每组20例。3组均给予常规抗结核、置管引流胸腔积液等综合治疗措施,尿激酶组在综合治疗的同时,胸腔内注射20万单位尿激酶,对照组胸腔内注射等容量的生理盐水,而尿激酶联合微波理疗组则在尿激酶组基础上辅以微波理疗。比较3组的临床疗效。结果胸腔内注射尿激酶联合微波理疗组胸腔积液消失时间为(8.5±5.2)d,显著短于对照组的(10.4±5.1)d和尿激酶组的(11.2±4.9)d,差异有统计学意义(P<0.05);治疗2个月后,胸腔内注射尿激酶联合微波理疗组胸膜厚度为(1.57±0.82)mm,明显小于对照组的(4.06±0.76)mm和尿激酶组的(2.33±0.85)mm,差异有统计学意义(P<0.01),而胸膜粘连发生率(30%)则明显低于对照组(80%)和尿激酶组(50%),差异也有统计学意义(P<0.05)。结论胸腔内注射尿激酶联合微波理疗治疗结核性包裹性胸腔积液疗效确切,且安全可靠,具有临床应用价值。
Objective To study the clinical value of microwave physical therapy combined with intrapleural urokinase on loculated tuberculous peural fluid. Methods Sixty patients hospitalized from June 2007 to May 2012 with loculatcd tuberculous peural fluid were divided into control group and urokinase group and microwave group, 20 cases in each. The three groups were all given systematic treatment measures such as anti - TB drugs and pleural drainage. Urokinase group was given extra intrapleural injection of urokinase. The control group was given the same amount of saline. The microwave group , besides the urokinase injection, was treated with microwave physical therapy. Results The duration of pleural effusion absorption of microwave physical therapy combined with intrapleural urokinase group was shorter than the urokinase group and the control group （P〈0.05）. The pleura was thinner and the incidence of pleural adhesion was significantly lower than the urokinase group and the control group （P 〈 0.05）. Conclusion Microwave physical therapy combined with intrapleural urokinase is an effective way for treating loculated tuberculous peural fluid.
