摘要
目的探讨术前外周血中性粒细胞与淋巴细胞比值（NLR）与胃癌各种临床病理指标的关系。方法分析201例胃癌患者术前NLR与肿瘤浸润深度、病理组织学特征、淋巴结转移及复发等各种临床病理指标的关系。结果胃癌术前的NLR与肿瘤浸润深度（χ2=6．147，P〈0．05）、淋巴结转移（χ2=28．297，P〈0．01）、术前血清癌胚抗原（CEA）水平（χ2=25．722，P〈0．01）、复发（χ2=43．620，P〈0．01）呈正相关，与肿瘤的组织学类型无明显相关。Kaplan．Meier生存分析表明NLR升高的胃癌患者生存时间为（28．448±3．395）个月，NLR正常组生存时间为（65．802±2．982）个月，预后明显差于NLR值低者（χ2=54．585，P〈0．01）。结论NLR可能是胃癌预后的重要指标，NLR是胃癌淋巴结转移及术后复发的指标之一。
Objective To find out the relationship between periferal blood neutrophil to lympho- cyte ratio and various clinicopathological factors in patients with gastric cancer. Methods A total of 201 patients with gastric cancer, who underwent curative resection in our hospital, were included. A total of 65 patients with neutrophil to lymphocyte ratio （NLR） of 2. 9 or more were classified as high NLR individuals, while the NLR of other patients was under 2. 9 in this study. Various elinicopathological factors including depth of invasion, pathological and histological characteristics, lymph node metastasis and tumor recurrence were analyzed retrospectively. Results Chis-Square test showed that the preoperative NLR was positively correlated with the depth of invasion, lymph node metastasis, recurrence and preoperative serum-careino- embryonic antigen（CEA） level, but not with the pathological and histological characteristics （ P 〉 0. 05 ）. Multivariate logistic regression analysis revealed that the lymph node metastasis, postoperative recurrence and preoperative serum-CEA level were the relevant factor of value of NLR among all the elinieopathologie variables. The Kaplan-Meier methodology indicated the prognosis of patients with a high NLR was signifi- cantly worse than that of patients with a low NLR （χ2 = 54. 585 ,P 〈 0. 01 ）. Conclusion The NLR is one of independent prognostic indicator, and it is one of independent factors as lymph node metastasis and post- operative recurrence in gastric cancer patients.
