上皮钙黏素及埃兹蛋白在胃癌和淋巴结转移灶中的表达及其意义 被引量:11

The expressions and significance of cytoskeleton-associated proteins in primary gastric carcinomas and the lymph nodal metastases

摘要 目的检测胃腺癌组织和淋巴结转移灶中细胞骨架相关蛋白上皮钙黏素(E-cad)及埃兹蛋白(Ezrin)的表达,观察其与胃腺癌侵袭和转移的关系,探讨胃腺癌的浸润和转移机制。方法用SP免疫组织化学法检测80例胃癌和40例淋巴结转移灶中E-cad和Ezrin的表达。结果E-cad和Ezrin在癌旁正常胃黏膜上皮为胞膜表达,而在胃腺癌组织中E-cad出现胞膜、胞质两种表达形式,Ezrin在肿瘤细胞中则为胞质表达。在胃腺癌原发灶中,E-cad、Ezrin的表达与胃腺癌的分化程度相关。E-cad的膜表达及Ezrin的表达随分化程度降低而下降(P〈0.01;P〈0.05),E-cad的浆表达随分化程度降低而升高(P〈0.01)。E-cad浆表达的升高与浸润深度、淋巴结状态相关且在原发灶显著高于转移灶(P〈0.01;P〈0.05;P〈0.01)。在淋巴结转移灶中,E-cad的膜表达在淋巴结转移的早期阶段高于晚期阶段(P〈0.05)。另外,E-cad膜表达及Ezrin的表达呈正相关(P〈0.01)。结论胃腺癌中E-cad的表达异常导致了细胞之间的黏附力下降,从而在胃腺癌的侵袭和转移中发挥重要作用。埃兹蛋白参与调节胃腺癌细胞的分化,可能通过与E-cadherin/catenin作用来调控肿瘤细胞的黏附和侵袭。 Objective To investigate the expression of E-cadherin and Ezrin in the primary gastric carcinomas versus lymph nodal metastases, and elucidate the relationship between these molecules and invasion and metastasis of gastric adenocarcinoma. Methods Eighty paraffin-embedded gastric carcinoma tissue samples together with 40 cases of lymph nodal metastases were obtained. Hematoxylin and eosin stain was used for reviewing and determining the patholoical diagnosis,including histological classification, tumor differentiation, depth of infiltration and nodal status. The expression of E-cadherin and Ezrin was detected by means of SP immunohistochemical method. Results Normal adjacent gastric mucosa showed intense membranous staining of E-cadherin and Ezrin throughout the epithelium. The staining patterns in carcinoma tissues were different from normal tissues. E-cadherin had membranous and cytoplasmic staining type. Cytoplasmic Ezrin staining was common in tumor cells. In primary lesions, the expression of E-cadherin and Ezrin was correlated with tumor differentiation. The expression of Ezrin and the membranous ex- pression of E-cadherin were reduced in those cases with higher grade of differentiation ( P 〈 0.05 ; P 〈 0.01 ,respectively). In contrast,the cytoplasmic expression of E-cadherin was increased (P 〈 0.01 ). The up-regulated cytoplasmic E-cadherin expression was associated with depth of infiltration and lymph node metastatic condition ( P 〈 0.01 ; P 〈 0.05, respectively ). The cytoplasmic expression level of E-cadherin was higher in primary tumor than in metatstases (P 〈 0.01 ). In lymph nodal metastases, the membranous expression of E-cadherin was suppressed in late stage of lymph node metastasis ( P 〈 0.05 ). In addition, a positive correlation was found between the expression of Ezrin and the membranous expression of E-cadherin ( P 〈 0.01 ). Conclusion The anomalous expression of E-cadherin leads to reduced cell-cell adhesion, thus plays a pivotal role in gastric cancer cells invasion and metastasis. Ezrin is involved in regulating differentiation of gastric cancer cells and regulates cell-cell adhesion and invasion of gastric adenocarcinoma by interacting with the E-cadherin/catenin complex.
