摘要
目的探讨腹腔镜卵巢囊肿剥除术中序贯式止血法对卵巢功能的影响。方法将68例患有单侧良性卵巢囊肿行腹腔镜卵巢囊肿剥除术的患者随机分为研究组和对照组,每组34例。研究组术中采用序贯式止血法,对照组术中采用传统的双极电凝止血。观察两组患者术前及术后3个月、术后6个月的雌二醇(E2)、促卵泡激素(FSH)、促黄体生成素(LH)激素水平。结果术后3个月两组患者血清E2均下降,FSH升高,与术前相比差异有统计学意义(P<0.05);术后6个月E2与FSH研究组与术前相比差异无统计学意义(P>0.05),对照组与术前相比差异有统计学意义(P<0.05),两组相比差异有统计学意义(P<0.05)。手术前后LH差异无统计学意义(P>0.05)。结论腹腔镜卵巢囊肿剥除术中采用序贯式止血法相对传统止血法,对卵巢功能损害小,序贯式止血能够更好地保护卵巢,值得临床推广应用。
Objective To explore the influence of sequential hemostasis on ovarian fianction during laparoscopic ovarian cystectomy. Methods 68 patients with ovarian cysts and underwent laparoscopic ovarian cystectomy were divided into research group and control group by random digits table method, with 34 cases in each group. The research group received sequential hemostasis, while the control group received conventional bipolar coagulation. Estradiol （E2）, follicle-stimulating hormone （FSH）, luteinizing hormone （LH） were measured in preparation, 3 and 6 months after operation. Results ①The serum E2 decreased and FSH increased in two groups 3 months after the operation, and compared with preoperative states, the differences were statistically significant （P 〈0.05）. ②Compared with preoperative states, the differences of E2 and FSH in research group were not statistically significant （P 〉0.05）, but the differences of E2 and FSH after operation in control group were statistically significant （P〈0.05）, and the differences of E2 and FSH between two groups were statistically significant （P 〈0.05）.③Compared with preoperative state, the difference of LH after operation was not statistically significant （P〉0.05）. Conclusions Sequential hemostasis is an effective method to protect ovarian function during the laparoscopic ovarian cystectomy. It is worthy of clinical application.
