心踝血管指数早期筛查2型糖尿病大血管病变的应用价值 被引量:2

摘要 目的研究心踝血管指数早期筛查2型糖尿病患者大血管病变的临床应用。方法选择2009年12月至2012年9月入院接受治疗的80例2型糖尿病患者作为研究组,另外选取30例非糖尿病患者作为对照,对所有受试者进行病史的采集,监测患者血糖生化指标,同时进行颈部动脉彩色多普勒超声检测,心踝血管指数(CAVI)测定,分析CA VI同颈动脉硬化的相关性。以CAVI为因变量,年龄、血压、体重指数、血脂、空腹血糖和糖化血红蛋白等指标为自变量进行多元线性回归分析。结果糖尿病组患者体重指数、甘油三酯、空腹血糖、糖化血红蛋白、颈动脉内膜中层厚度(IMT)、CAVI值及颈动脉斑块发生率均高于非糖尿病组,两组差异有统计学意义(P<0.05)。CAVI同受试者的IMT呈正相关,多元线性回归分析结果CAVI同年龄,甘油三酯,空腹血糖值,HbA1C存在显著正相关。结论CAVI同糖尿病患者的IMT呈正相关,早期进行CAVI的测定可以预测大血管的病变。 Objective To observe the value of heart ankle vascular index in early screening of vascular disease in patients with type 2 diabetes.Methods Eighty patients with type 2 diabetes mellitus from December 2009 to September 2012 were selected as treatment group and 30 cases of patients without diabetes were chosen as control group.The disease history of the cases in two groups were collected,the blood sugar biochemical indicators monitored,the neck artery color doppler ultrasonic testing and CAVI measurement peroformed,CAVI with carotid atherosclerosis correlation.CAVI as the dependent variable,age,blood pressure,body mass index,blood lipids,fasting blood glucose and glycosylated hemoglobin indicators as independent variables for multiple linear regression analysis.Results Diabetes patients body mass index,triglyceride,fasting plasma glucose,glycosylated hemoglobin,carotid intima-media thickness,CAVI value and carotid artery plate rate were higher than those of non-diabetic group,showing significant difference.CAVI was positively correlated with the IMT of subjects.Multiple linear regression analysis indicated that CAVI was associated positively with age,triglycerides,fasting glucose,HbA1C.Conclusion CAVI was positively correlated with IMT in patients with diabetes and early CAVI measurement can predict large vascular lesions.
