摘要
目的评价复合乳酸菌胶囊对三联疗法根除消化性溃疡患者幽门螺杆菌感染疗效的影响。方法本研究为随机、阳性对照研究。100例胃镜检查和病理证实为患消化性溃疡的患者,14C呼气实验证实幽门螺杆菌现症感染,且从未经历根除幽门螺杆菌治疗。对照组:50例患者接受根除幽门螺杆菌标准三联疗法,奥美拉唑20mg,阿莫西林1000mg,克拉霉素500mg,全部药物口服2次/d,疗程为7d。治疗组:50例患者除接受标准三联疗法外,同时联用复合乳酸菌胶囊0.66g2次/d,服药7d。治疗结束后6周,使用14C呼气实验评价疗效。结果两组幽门螺杆菌成功根除率(90%vs86%,P=0.538)和消化性溃疡愈合率(93.5%vs90.3%,P=0.674)差异无统计学意义。治疗组和对照组不良反应发生情况:腹泻发生率(8%vs24%,P=0.029)、腹胀发生率(6%vs20%,P=0.031)、恶心发生率(10%vs26%,P=0.037)差异无统计学意义。结论根除幽门螺杆菌标准三联疗法联合复合乳酸菌提高幽门螺杆菌的根除率,但可以减轻不良反应,提高患者的治疗依从性。
Objective To evaluate the effect influence of compound lactobacillus capsule combining with triple therapy in curing Helicobacter pylori （Hp） in peptic ulcer patients. Methods In the study,the randomized positive control trial was carried out.100 patients with peptic ulcer were confirmed by endoscopy and pathology and their HP infections were proved by 14 C breath test.All participants didn’t ever accept the radical treatment of Hp and were evenly divided into control group and treatment group.In the control group,in order to cure Hp,the standard triple therapy containing omeprazole 20 mg,amoxicillin 1000 mg,clarithromycin 500 mg was provided and all drugs were orally administered twice per day for 7 consecutive days.In the treatment group, on the basis of the control group,compound lactobacillus capsule was added with the dosage of 0.66 g twice per day lasting for 7 days.6 weeks after treatment,the curative effect was evaluated by 14 C breath test. Results The Hp eradication rates in the treatment group and control group were 90% and 86% and the healing rates of peptic ulcer were 93.5% and 90.3% respectively.The two parameters didn ’t display statistical differences （P=0.538 vs P=0.674）.With the respect of adverse reaction,the incidences of diarrhea （8% vs 24%, P = 0.029）,abdominal distension （6% vs 20%,P=0.031）,and nausea （10% vs 26%,P=0.037） displayed significant differences in comparison of the two groups. Conclusion In the radical treatment of Hp,the standard triple therapy combining with compound lactobacillus capsule doesn’t increase the eradication rate,but it can reduce the occurrence of adverse reaction and improve patient’s compliance during treatment.
