摘要
目的探索制订左氧氟沙星注射剂临床合理使用评价标准,为临床合理使用左氧氟沙星注射剂提供参考。方法依据药品说明书和卫生部有关规定等,建立左氧氟沙星注射剂临床合理使用评价标准,然后随机抽取某三甲医院使用左氧氟沙星注射剂住院患者出院病历120例,进行评价,发现存在问题。结果建立的左氧氟沙星注射剂临床合理使用评价标准,包括适应证、联合用药、管理指标、用法、用量、药物的配伍禁忌及相互作用、临床疗效及疗效监测措施等内容。通过回顾性应用评价标准,发现某院左氧氟沙星注射剂在临床应用过程中做得较好的方面有:联合用药、给药剂量和给药间隔、给药途径、处方医师权限管理均符合标准;但存在用药疗程不足或偏长现象、不符合适应证标准比例占29.17%、14.17%、存在药物配伍禁忌、滴注时间在病例中无记录等不合理现象。结论建立的左氧氟沙星注射剂临床合理使用评价标准在评价左氧氟沙星注射剂临床使用合理性方面具有较高的科学性和权威性,能够发现临床用药存在的问题。
Objective To explore and establish levofloxacin injection clinical rational use evaluation standard so as to provide preference for its clinical use. Methods 120 discharge records of inpatients from one top three hospital were randomly selected to evaluate its clini- cal levofloxacin injection use with the standard formulated on the basis of medicine specification and relevant rules of the Ministry of Health. Results The newly formulated standard for the rational use of levofloxacin injection contained indications, drug combination, management indicators, usage, dosage, drugs incompatibility, drugs interaction, clinical efficacy, and efficacy supervision, etc. It was dis- covered that the hospital has done good in the following aspects according to the standard, drugs combination, dosage, dosing interval, ad- ministration route, and prescription physician rights management; however, some irrational uses, such as treatment course deficiency or o- verlong, unmatched drug use and indications, accounting for 29.17% ,and problems such as drugs incompatibility,unrecorded intravenous time accounted for 14.17%. Conculsions The newly established clinical rational use evaluation standard of levofloxacin injection shows high scientificity and authoritativeness, which will be helpful to detect more problems existing in clinical drug uses.
