摘要
目的探讨改良水化方案在预防高龄患者造影剂肾病改善患者焦虑情绪的临床疗效。方法选取来我院行介入治疗的患者90例,随机平均分成对照组和改良组。对照组给予常规水化治疗。治疗组给予改良水化治疗。两组患者均在术前后测定血清肌酐含量以评定肾脏功能,均在介入术前后进行抑郁自评量表(SDS)和焦虑自评量表(SAS)评分,并根据得分判定患者情绪状态。结果介入术后,(1)第1 d,第2 d,第3 d,改良组血清肌酐明显低于对照组,差异有统计学意义(P<0.05)。(2)改良组有1例(2.2%)患者出现造影剂肾病,对照组有6例(13.3%),两组造影剂肾病发生率比较差异有显著统计学意义(P<0.01)。(3)介入术后,改良组的SAS评分(38.6±2.5)与对照组(45.6±2.5)相比,差异有统计学意义(P<0.05)。结论改良水化方案对预防高龄患者造影剂肾病和改善患者焦虑情绪的临床疗效显著,值得推广。
Objective To observe the clinical efficacy of prevention of contrast-induced nephropathy and reliving anxiety through improved hydration treatment in elderly patients.Methods 90 elderly patients who were treated interventional therapy in our hospital were taken.They were randomly,equally divided into control group and improvement group.The control group was given conventional hydration therapy.The treatment group was given improved hydration therapy.The serum creatinine of all patients was tested to estimate the renal function.The patients of two groups were scored according to SDS and SAS at the time of pre-treatment and post-treatment.The emotional state was judged by the score.Results After interventional therapy,1） first days,second days,third days,the increment of serum creatinine in improved hydration treatment group was significantly lower than that of the control group.There was statistical difference（P＆lt;0.05）.2） There was 1 case（2.2%） with contrast nephropathy in improved hydration treatment group,6 cases（13.3%） in the control group.The incidences of contrast-induced nephropathy in two groups were statistically significant（P＆lt;0.01）.3） SAS score（38.6±2.5） of improved hydration treatment group was significantly lower than control group（45.6±2.5）.There was statistically significant difference（P＆lt;0.05）.Conclusion Improved hydration treatment had clinical preventive effect in contrast-induced nephropathy and reliving anxiety in elderly patients,and it was worthy of promotion.
