不同病程2型糖尿病患者的肾损伤特点 被引量:2

Characteristics of Renal Injuries in Type 2 Diabetes with Different Duration of Disease

摘要 目的观察不同病程2型糖尿病患者血胱抑素C(CysC)、血β2微球蛋白(β2MG)、24h尿白蛋白排泄率(UAER)、血清肌酐(Scr)、尿素氮(BUN)变化,探讨不同病程2型糖尿病患者肾损伤的特点。方法 200例糖尿病患者按病程分为病程<5年组、5~10年组及>10年组,以45例健康体检者为对照组,检测受试对象CysC、β2MG、UAER、Scr、BUN等肾脏代谢指标;检查受试对象空腹血糖(FPG)、空腹C肽(C-P)、空腹胰岛素(Insulin)、糖化血红蛋白(HbA1c)等糖代谢指标,并计算HOMA-IR。统计分析各组检测指标差异。结果糖尿病各组随着病程延长,CysC、β2MG逐渐升高,各组差异有显著性;病程5~10年组及>10年组UAER显著高于健康对照组、病程<5年组;Scr、BUN、HbA1c在病程>10年组显著升高;C-P在病程>10年组显著下降;糖尿病各组FPG、FINS、HOMA-IR无显著性差异;血Cysc、β2MG与UAER呈直线相关。结论不同病程2型糖尿病患者存在不同程度肾损伤,血清CysC、β2MG随病程延长逐渐升高,能更早地反映糖尿病患者肾脏损伤。 Objective To study the changes of serum cystatin c(Cys c), β2-microglobulin(132MG),24 hour uria albumin exertion rate(UAER), blood urea nitrogen (BUN)and creatinine of patients with type 2 diabetes in various course of diseases, and to observe the characteristics of renal injury in diabetic patients. Method 200 patients with type 2 diabetic were divided into three groups according to course of diseases less than 5 years,5-10 years and more than 10 years, 45 healthy cases as the control group. We measured the serum cystatin c, β2MG, UAER, creatin, BUN, fasting glucose(FPG), fasting C peptide(C-P) ,fasting insulin,HbA 1 c and HOMA-IR of all cases.Result The cys c and β2MG of all diabetic groups were gradually increasing with extending course, the UAER of the group which course was 5-10 years and more than 10 years had significantly difference than control group and less than 5 years group; Scr. BUN. C-P, HbAlc of group with more than 10 years course of diseases had significantly differentce between over three groups.There was positive correlation between cystatin c and β2MG with UAER. Conclusion There was different degree renal injuries in type 2 diabites with different duration of disease. Serum cystatin c and β2MG of patients with type 2 diabetes mellitus was gradually increasing with extending course and could reflect renal injury of diabetic patients more earlier.
