原发性高血压患者脂蛋白a水平与踝臂指数的关系

Relationship Between Serum Level of Lipoprotein a and Ankle-brachial Index in Patients with Primary Hypertension

摘要 目的观察原发性高血压患者脂蛋白a[LP(a)]水平,并探讨其与踝臂指数的关系。方法 2011年1月至2011年12月间就诊的160例原发性高血压患者为观察组,80例健康体检者作为对照,采用免疫比浊法检测血清LP(a)水平,比较组间差异。测量观察组患者踝臂指数(ABI),比较LP(a)升高组与LP(a)正常组ABI差异,并对LP(a)及ABI行直线相关分析。结果观察组LP(a)水平{[354(127,396)]mg/L vs.[162(103,214)]mg/L,u=7.335}及升高者比例(35.6%vs 13.8%,χ2=12.568)显著高于对照组,差异有统计学意义(P<0.05)。与LP(a)正常组比较,LP(a)升高组ABI降低者比例显著升高(59.6%vs.31.1%,P<0.05),而ABI值显著降低[(0.7±0.4)vs(1.2±0.5),t=3.149,P<0.05]。直线相关分析显示血清LP(a)水平与ABI值呈负相关(r=-0.416,P<0.05)。结论原发性高血压患者中血清LP(a)水平高于正常人群,其水平与ABI呈负相关。 Objective To observe the level of serum lipoprotein a [LP(a)] in patients with primary hypertension, and to explore the relationship between serum level of LP(a) and ankle-brachial index(ABI). Methods From Jan. 2011 to Dec. 2011, a total of 160 patients with primary hypertension were enrolled in observation group, Serum level of LP(a) was detected by immunoturbidimetry, and compared with 80 healthy controls. ABI of patients in observation group were detected, and the difference between high LP(a) level and normal LP(a) level were compared. Linear correlation analyze was used to analyzed the relationship between LP(a) and ABI. Results Both of level of serum LP(a) {[354(127,396)] mg/L vs.[ 162(103,214)] mg/L ,u=7.335}and high LP(a) ratio in observation group (35.6% vs. 13.8%,X2 =12.568) were significantly higher than those in control group(P〈0.05). Compared with normal LP(a) group, the retio of low ABI was significantly higher(P〈0.05), and ABI was significantly higher in high LP(a) group[(0.7±0.4) vs. (1.2±0.5), t=-3.149, P〈0.05]. Linear correlation analyze showed that the Lp(a) was negatively related with ABI(r=-0.416,P〈0.05). Conclusion The level of serum LP(a) was higher in patients with primary hypertension than that in healthy people, and it was negatively related with ABI.
