依托咪酯、咪唑安定用于休克患者全麻诱导的比较

摘要 目的 :本研究拟了解依托咪酯与咪唑安定作为全麻诱导剂对休克患者心血管系统的影响 ,同时对其不良反应进行观察。方法 :选择 36例需急诊手术的休克患者 ,分为两组 :依托咪酯组 (E组 ,18例 )采用依托咪酯、芬太尼及司可林进行全麻诱导 ;咪唑安定组 (M组 ,18例 )则采用咪唑安定、芬太尼及司可林。观察患者的麻醉诱导时间、诱导前后及气管插管后 1分钟、 3分钟、 5分钟和 10分钟的心律、心率、血压、脉搏、氧饱和度、呼吸变化及术后清醒等情况。结果 :E组诱导前后及插管后 ,心率血压均无明显变化 ,M组插管后心率血压有一过性升高 ,两组的副作用均较小。结论 :对于休克患者而言 ,依托咪酯和咪唑安定均是较为安全有效的全麻诱导剂 ,其中 ,依托咪酯对心血管系统影响更小 ,用药安全性更高。 Objective: The purpose of this study was to compare the cardiovascular response to shock patients in anesthesia induction using etomidate or midazolam, simultaneously to observe the side effect.Methods:36 emergency shock patients were divided into two groups (n=l8). One is Etomidate group (Group E) in which etomidate, fentanyl and scoline were used in anesthesia induction, another is Midazolam group (Group M ) in which midazolam, fentanyl and scoline were used. The induction time and the status of post—operation awake were observed. We took the data of HR, BP,Pu1se, breath and SpO2 at a base state, after induction and l,3,5 and l0 minutes after orotracheal intubation. Results: In Group E, there were no distinguishable change of HR and BP before/after induction and after intubation. In Group M, after intubation HR and BP had been detected an increasein a flash. In both groups, there were no obvious side effects. Conclusion : To the shock patients,etomidate and midazolam are all effective and safety in anesthesia induction. But etomidate has less cardiovascular response. So, etomidate is more safety than midazolam as a revulsant. \ \
