摘要
目的探讨2型糖尿病患者肾小球滤过率(GFR)的变化与其视网膜病变(DR)分期的关系。方法对248例2型糖尿病患者进行回顾性研究,根据眼底检查分成正常眼底(NR)、背景型糖尿病视网膜病变(BDR)和增殖型糖尿病视网膜病变(PDR)三组,测定各组的24小时尿白蛋白及GFR。结果与正常眼底组比较,DR组的高血压患病率、FPG、PPG、HbA1C、Scr、Tch、LDL-D、24小时尿白蛋白均升高(P<0.05),GFR下降(P<0.05);24小时尿白蛋白、GFR均与DR正相关。同时,在GFR正常的2型糖尿病患者中DR的发病率为36%;随着GFR下降,DR的发病率增加,且程度加重;当GFR<60ml/(min.1.73m2)时,DR的发病率上升至84.7%。结论 24小时尿白蛋白、GFR与DR密切相关,其联合筛选有利于DR的早期诊断。
Objective To discuss the relationship between the change of urine albumin and the glomerular filtration rate (GFR) of type 2 diabetes mellitus and its stage of diabetic retinopathy (DR) . Methods 248 cases of type 2 diabetes mellitus were retrospectively studied, and the 24 hours urinary albumin and GFR were tested in three groups of normal retinal (NR), background diabetic retinopathy (BDR) and proliferative diabetic retinopathy (PDR), according to the fundus exami- nation. Results Compared with the normal retinal group, hypertension prevalence, FPG, PPG, HbA1C, Scr, Teh, LDL- D, 24 hours urinary albumin of DR group were significantly increased (P 〈 0. 05 ) while GFR decreased obviously (P 〈 0.05 ) ; 24 hours urinary albumin and GFR were significantly related to DR. At the same time, the incidence of DR in GFR normal patients with type 2 diabetes was 36%. With the decline in GFR, the incidence of DR increased and aggravated. When GFR was less than 60 ml/ ( min· 1.73m^2 ), the incidence of DR rose to 84. 7%. Conclusion 24 hours urinary albumin and GFR are closely related to the incidence of DR, and joint screening is conductive to early diagnosis of DR.
