摘要
作者采用犬肾动脉狭窄高血压(RVH)模型24只,临床患者15例,系统观察了尿丙氨酸氨基肽酶(AAP),N-乙酰-β-葡萄糖苷酶(XAG)及r-谷氨酰转肽酶(GGT)的变化。结果显示:犬RVH形成后4-8周,尿AAP从1.8U/gCr升到5.3U/gCr;NAG从9.4U gCr到26.9U gCr;尿GGT早期升高,后期下降。12—16周后动脉未狭窄侧肾尿酶亦升高。15例RVH患者中13例尿AAP,NAG排泌值显著性升高,超过正常上限;而42例原发性高血压的尿酶值超过正常上限者仅8例,阳性率分别为86.8%和19.1%(P<0.001)。尿蛋白检测两组间无统计学差异。尿酶升高是RVH肾组织早期损伤的指征。检测尿AAP,NAG可以作为RVH的筛选性试验。
This article aimed at evaluating the value of urinary enzymes as a screening test for renal vascular hypertension ( RVH ) in patients with hypertension . Urinary alanme aminopeptidase (AAP), N- acetyl - beta - D - glucosaminidase ( NAG ) excretion increased in dogs with Goldblatt's hypertension 4-8 weeks after eliciting high blood pressure (AAP from 1.8 to 5.3 U gCr, NAG from 9.4 to 26.9 U/gCr); while the activity of r - glutamyltransferase (GGT ) increased only 2 - 8 weeks alter the operation ( from 0.9×103 to 19 ×increase of AAP (28.5 U/gCr), NAG (25.9 U gCr) and slight rising of GGT (0.8×103 U/gCr) were observed in patients with RVH . The patients with elevated enzymuria were found in 13 (86.8% )out of 15 RVH and 8 (19.1% )of 42 patients with essential hypertension ( P<0.001 ); but there are no difference between proteinuria . The result showed that the rising of AAP , NAG in urine might be taken as an early sign of renal damag, Urinary AAP , NAG may be examed as a screening test for RVH in patients with hypertension .
