摘要
目的:探讨静脉移植在断指再植中的临床疗效。方法:在放大10倍的手术显微镜下对离断指体彻底清创,显露指动静脉指神经,按照常规固定骨折修复肌腱,再行腕掌侧静脉移植,分别行动脉远、近断端吻合,通血后行指神经及静脉吻合。伴皮肤缺损行转移或邻指皮瓣修复。结果:28例31指,成活30指,成活率为96.8%。术后随访6～12个月见再植指体外形饱满,痛温觉恢复,指腹两点分辨觉8～1.0mm,患指功能恢复良好。结论:静脉移植在断指再植中的应用,使复杂性损伤严重的断指再植成活率得到了较大的提高,提高了优良率。
Objective：To explore the clinical results of vein graft in the replantation of amputated fingers.Methods：The amputated part was debrided under 10X high power magnification.Unfold the fingers artery and vein and nerve.Kirschner wires were uniformly used for bony fixation.Tendon repair was carried out next.Carpometacarpal superficial vein graft was used for rehabilitated artery,And then repaired fingers nerve and vein after blood circulated.Transfer flaps or handy finger island flaps were used to repair skin defects of the fingers. Results：Twenty - eight cases（thirty - one fingers）,thirty fingers were survived.The success rate of replantation was 96.8%.Follow up period ranged from 6 to 12 months was carried out and showed that the successfully replanted fingers had good soft - tissue coverage and recovery of pain and touch sensation.Two - point discrimination of the pulp was 8 to 10 mm.The fingers had fine flexion and extension function.Conclusion：The application of vein graft in the replantation of amputated fingers could be patency enhance the survival rate and the excellent rate of the complexity and serious replanted finger.
