摘要
目的 总结股骨下段骨折合并的血管神经损伤处理的经验教训 ,以降低伤残率。方法 血管伤 78例 ,神经伤 37例 ,其中 19例系血管神经同时受伤。血管伤分别行血管减压和扩张 ,血管修补 ,血管吻合 ,血管移植术。神经损伤分别行解除压迫 ,神经吻合 ,神经移植术。结果 血管伤 78例 ,6 9例恢复血运 ,9例截肢。神经伤 37例 ,除 9例截肢外 ,4例未恢复神经功能外 ,其余全部恢复神经功能。结论 对血管神经损伤 ,要及时准确地做出诊断 ,及时处理。对血管损伤严重者 ,要尽早行血管移植。对神经损伤 ,要早期探查 ,严重者 ,要行神经移植术。
Abjective To summarize the experience and learn the lesson from early repair of subfemur fracture associated with vascular and nerve injury,so that to reduce the rate of disability. Method To repair vascular injury by the way of vasotransplantation,vasodepressor,vasodilatation,vasorepair and vasoanastomosis.To repair nerve injury by neurolysis, neuroanastomosis and neurotransplatation.Result Among 78 patients with vascular injury,69 cases survived after operation,9 cases had undergone amputation.Among 37 cases with nerve injury,24 cases had recovered the function of nerve after operation except 13 cases.Conclusion To associated vascular and nerve injury of subfemur fractures,an accurate diagnosis and treatment in time is important.For severe vascular and nerve injury,vasotransplantation or neurotransplantation in time is also necessary.
