首次发病重性抑郁障碍患者治疗前后大脑灰质体积变化的结构磁共振研究 被引量:2

摘要 目的用结构磁共振手段对首发未用药的重性抑郁障碍患者治疗前后大脑灰质体积的变化进行研究。方法对21例重性抑郁障碍患者分别在治疗前后进行脑结构磁共振扫描,应用基于体素的形态学测定法,比较重性抑郁障碍患者治疗前后大脑灰质体积的差异。结果与治疗前相比,治疗后MDD患者左侧海马、右侧背外侧前额叶灰质体积显著增加,左侧丘脑灰质体积显著减小。结论治疗前后重性抑郁障碍患者存在边缘系统-皮层-丘脑神经环路相关脑区结构的动态变化,说明重性抑郁障碍患者存在多个脑区的脑结构异常,并且这种异常能在有效抗抑郁治疗后随着症状缓解而改善,抑郁症患者的边缘系统-皮层-丘脑神经环路结构异常可能在抑郁症的发病机制中发挥重要作用。 Objective To explore brain volumetric changes of antidepressants in patients with major depressive disorder using structural magnetic resonance imaging. Methods Structural magnetic resonance imaging data were ac- quired from 21 MDD patients. The grey matter volume was compared before and after antidepressant therapy in MDD patients. Results After antidepressant treatment, patients showed increased grey matter volume in left hippocampus and right dorsolateral prefrontal cortex and decreased grey matter volume in left thalamus. Conclusion MDD patients show the dysfunction in several brain regions related to the limbic-cortical-thalamus circuits, and can be improved by antidepressant treatment, which may play a key role in the pathophysiology of MDD.
