MRI与能谱CT在诊断手肌腱病变中的临床应用价值 被引量:3

摘要 目的探讨MRI与能谱CT在诊断手肌腱病变中的临床应用价值。方法选取2011年1月至2012年9月在深圳市龙岗中心医院手外科专科医院就诊的50例手肌腱疾病患者和20例手肌腱正常者行MRI和能谱CT检查,比较MRI和能谱CT在手肌腱正常解剖及基本病变中的影像学表现。结果 MRI在显示手肌腱变性、粘连、腱鞘病变方面较能谱CT有明显优势(P<0.05);能谱CT在显示手肌腱的形态、走形及止点方面,特别是在显示骨骼、肌腱及手肌肉的三维关系方面有明显优势;MRI和能谱CT在显示肌腱断裂、缺失及增粗方面无明显差异(P>0.05)。结论 MRI和能谱CT在诊断手肌腱病变中有各自的优势,应根据不同的临床诊断目的选择合适的检查方法。 Objective To study the clinical application values of magnetic resonance imaging (MRI)and spectral computed tomography(CT) on hand tendon injury. Methods Fifty patients with hand tendon injury and 20 persons with other diseases from Hand Surgery Hospital of Longgang Central Hospital of Shenzhen during Jan. 2011 and Sep. 2012 were detected by MRI and spectral CT, and the imaging manifestations of M RI and CT of hand tendon anatomy and basic lesion were compared. Results MRI was significantly superior to spectral CT in showing tendon degeneration, adhesion, and tendinous sheath disease ( P 〈 0.05 ). Spectral CT was significantly superior to MRI in revealing the shape, abnormal shape, and stop point, especially in revealing the three-dimensional relation-ship of skeleton, tendon and muscle. However, there were no statistical significance in showing tendon rupture, absence and thickening between MRI and spectral CT(P 〉 0.05 ). Conclusion MRI and spectral CT have their respective advantages. In clinical decisions, appropriate method should be chosen based on clinical diagnosis pur-pose.
