摘要
目的:研究甲泼尼松龙辅助治疗肺炎支原体肺炎的效果及其免疫影响。方法:2008年1月～2012年12月唐山市妇幼保健院支原体肺炎患儿,采用随机数字法均分为两组,根据是否接受甲泼尼松龙辅助治疗分为观察组与对照组。对两组患儿治疗的临床效果,及治疗前后血清CRP、IL-2、IL-8、TNF-α水平进行比较。结果:观察组治疗显效率明显高于对照组(57.0%vs.28.0%,P<0.05);其无效率则低于对照组(5.4%vs.19.4%,P<0.05)。甲泼尼松龙辅助治疗支原体肺炎前后,血清CRP、IL-8、TNF-α水平降低,IL-2水平升高(P<0.05);甲泼尼松龙辅助治疗支原体肺炎后,血清CRP、IL-8、TNF-α水平降低,IL-2水平升高,与对照组治疗后相比较,差异均有统计学意义(P<0.05)。结论:甲泼尼松龙辅助治疗支原体肺炎能够较快的减轻肺部炎症反应,缓解临床症状,缩短治疗时间,但远期效果尚待进一步随访。
Objective： To study the effect of methylprednisohme for adjunctive therapy of mycoplasma pneumonia and its immune effect. Methods： The children with mycoplasma pneumonia were selected from the hospital from January 2008 to December 2012, then they were divided into observation group and control group according to receiving adjunctive therapy with methylprednisolone or not by random number method; the clinical curative effects, serum levels of C - reactive protein （CRP） , IL - 2, IL - 8, and tumor necrosis factor - α （TNF - α） before and after treatment in the two groups were compared. Results： The obvious effective rate in observation group （57. 0% ）was statistically significantly higher than that in control group （ 28.0% ） （ P 〈 0. 05 ） ; the ineffective rate in observation group （5.4%） was statistically significantly lower than that in control group （ 19.4% ） （P 〈 0. 05） . After treatment, the serum levels of CRP, IL - 8, and TNF - cL ,~ecreased, the serum level of IL - 2 increased （P 〈 0. 05 ）, compared with control group, there was statistically signifi- cant difference （ P 〈 0. 05 ） . Conclusion ： Methylprednisolone used for adjuvant therapy of mycoplasma pneumonia can relieve pulmonary inflammatory response rapidly, alleviate the clinical symptoms, and shorten the treatment time, but the long - term effect need further follow - up.
