摘要
目的:研究中医药孕前调治对BA低下性RSA患者的临床疗效。方法:选取BA低下性RSA患者111例,随机分为3组:中药组41例,孕前给予自拟益肾毓麟汤治疗;中药联合免疫组38例,孕前给予自拟益肾毓麟汤,同时接受配偶淋巴细胞体外诱生免疫疗法治疗;空白组32例,孕前未采取任何调治措施,自然妊娠。比较中药组和中药联合免疫组治疗前后抗CD3-BE、抗CD4-BE、抗CD8-BE、抗CD4-BE/抗CD8-BE的变化及3组的妊娠结局。结果:①中药组和中药联合免疫组治疗后抗CD3-BE、抗CD4-BE均有所上升,其中抗CD4-BE较治疗前显著上升(P<0.05),两组封闭效率上升无明显差异(P>0.05)。②中药组和中药联合免疫组成功妊娠率均高于空白组(P<0.05)。③成功妊娠组抗CD4-BE较治疗前显著上升(P<0.05),抗CD8-BE较治疗前显著下降(P<0.05),此种改变中药组较中药联合免疫组更为显著(P<0.05)。结论:中医药孕前调治可明显提高BA低下性RSA患者的封闭抗体水平,改善其妊娠结局,其作用较中药联合免疫治疗更为突出。
Objective ：To observe the clinical effect on the recurrent spontaneous aborters with insufficient blocking antibodies treated with traditional Chinese medicine during pre - pregnancy. Methods ： We divided 111 recurrent spontaneous aborters with insufficient blocking antibodies into 3 groups randomly：the TCM group, the TCM combined with immunotherapy group and the blank group. The TCM group were treated with Yishen Yulin Decoction, the TCM combined with immunotherapy group were immunized with the in - vitro inducing pate.rnal lymphocytes, meanwhile, they were treated with Yishen Yulin Decoction. The blank group got pregnant naturally with no treatment. The change of the blocking anti- bodies level and the pregnancy outcome after treatment were observed. Results：（1）The blocking efficiency on CD3 and CD4 was increased, among them the level of the anti - CD4 blocking antibody was increased significantly（ P 〈 0.05）, but there was no obvious difference on the improvement of the blocking efficiency between the two groups （P 〉 0.05 ）. （2）The successful pregnancy rate of the TCM group and the TCM combined with immunotherapy group was obviously higher than that of the blank group（ P 〈 0.05 ）. （3）The blocking efficiency on CD4 of those with successful pregnancy was obviously higher after treatment（ P 〈 0.05 ） ,while that on CD8 was obviously lower after treatment（ P 〈0.05 ） ,this improvement of the TCM group was more significant than that of the TCM combined with immunotherapy group （ P 〈 0. 05 ）. Conclusion： Traditional Chinese medicine treatment during pre - pregnancy can improve the blocking efficiency of the recurrent spontaneous aborters with insufficient blocking antibodies significantly, and raise the rate of successful pregnancy, which has a more outstanding effect than the TCM combined with immunotherapy.
