摘要
目的:本研究旨在观察高血压病(EH)及糖尿病(DM)患者肾动脉血流的彩色多普勒速度能量图(CCD)变化并探讨其影响因素。方法:以正常对照组及EH组,DM组为研究对象,每组各60例,均为男30例,女30例,对照组年龄37~75岁,EH组40~77岁,DM组41~73岁,采用ACUSON公司生产的ASPEN彩色多普勒超声显像仪,探头频率4~2MHZ及3.5MHZ,应用CCD测量肾动脉主干最大血流速度(Vmax),最小血流速度(Vmin),血管阻力指数(RI),肾血流量(CO),并检测心脏大小及血肌酐等。结果:EH组RI明显增高,CO及Vmax减低,DM组RI明显增高,CO及Vmax明显减低,EH及DM组的肾脏彩色血流信号稀疏,血管数量减少.血管内径变细,管壁增厚。当EH及DM患者的血肌酐正常时,CCD检测的肾血流多项指标改变较血肌酐更敏感。结论:应用CCD检测对EH及DM并发肾脏损害的及时诊断和治疗提供了更敏感、更准确的信息,且无创、快速、可以重复使用。
Objection: This research could directly ohaerved the changes of the color Doppler Energy and Velocity image of the renal artery flow and determine actores of these affected changes in EH and DM. Method: Divided into these groups: normal contrust, hyHertention (EH ) and Diabetes (DM) groups. Each group had 60 cases(male and female half). Used color Doppler ultrasonic(Aspen, Acuson made, detective head frequences 4~2 MHZ and 4 ~3. 5 MHZ), We examined the maxmial Velocity (Vmax), the minimum Velocity(Vmin), the resistence index(RI). the blood flow volurne of the main renal artery and examined the diameter of heart and tested blood creatinine in same time. Result EN group had high RI, low renal blood volume and Vmax. The affictive actors may were the ages, sexual, pulse rate, left ventricle weight index(LVWI),systolic artery presaure, blood creatinine. DM group had high RI, low renal blood volume and Vmax; the affective actors may be the ages, sexual, pulse rate,LVWI, blood sugar level and creatinine. EH and DM groups had the weaker nefleetive waves in the renal flow, fewer in blood vessel numbers smaller in the diameter of blood vessel, thicker in the blood wall. The changes of multiple parameters of renal flow were more sensitive than the changes of blood Cr.. Conclusion: Applied in CCD could detected the damages of the renal in the EH and DM cases in real-time and gave the senser and more exactly help in the clinic treatment, It was a non-damage, real time repeat method
