摘要
目的:探讨慢性肺源性心脏病(肺心病)患者血清超敏CRP(hs-CRP)和N末端B型利钠肽原(NT-proBNP)测定的临床价值。方法:将98例COPD患者分为单纯COPD组(A组,31例),COPD并发代偿期肺心病组(B组,35例),COPD并发失代偿期肺心病组(C组,32例)。检测3组及C组治疗后的血清hs-CRP和NT-proBNP水平。结果:3组血清hs-CRP和NT-proBNP水平依次为:C组>B组>A组,两两比较差异均有统计学意义(P均<0.01);C组治疗后血清hs-CRP和NT-proBNP水平均较治疗前明显下降(P均<0.01)。结论:血清hs-CRP和NT-proBNP水平均为鉴别COPD患者是否合并肺心病及其严重程度的有用指标,动态观察hs-CRP和NT-proBNP的水平变化对评价COPD并发肺心病患者的疗效有重要价值。
Objective： To explore the clinical value of serum high sensitivity C-reactive protein （hs-CRP） and N-terminal pro-B-type natriuretie peptide （NT-proBNP） detected in chronic pulmonary heart disease. Meth- ods ： Ninety-eight patients with chronic obstructive pulmonary disease （COPD） were divided into 3 groups. Group A contained 31 cases who had COPD merely. Group B contained 35 cases who had COPD, accompanied chronic pulmonary heart disease during compensatory stage. Group C contained 32 cases who had COPD, by chronic pul- monary heart disease during decompensatory stage. The serum hs-CRP and NT-proBNP were detected in 3 groups and group C after treatment. Results ： There were significant differences among serum hs-CRP and NT-proBNP lev- els of these three groups （ C group 〉 B group 〉 A group, all P 〈 0.01 ）. Compared with other two groups, the ser- um hs-CRP and NT-proBNP in group C decreased obviously after treatment （P 〈 0. 01 ）. Conclusion： The levels of serum hs-CRP and NT-proBNP are useful in differentiating COPD by pulmonary heart disease or not, and their severity. Observing serum hs-CRP and NT-proBNP movingly have important clinical value for assessing the thera- peutic effect of COPD by pulmonary heart disease patients.
