速度向量成像技术评价高血压患者左室扭转运动

摘要 目的:应用速度向量成像(VVI)技术评价高血压患者左室扭转运动。方法:对20例健康成人和58例原发性高血压患者行常规二维超声心动图检查后,根据左室壁厚度及二尖瓣口血流E/A值,将原发性高血压患者分为3组:单纯高血压组(20例)、高血压伴左室舒张功能下降组(20例)、高血压伴左室肥厚组(18例)。进行采集并存贮3beats状态下心底部短轴切面和心尖部短轴切面二维超声动态图,在VVI模式下对受检者勾画心内膜,应用VVI软件自动跟踪分析左室扭转角度。结果:与正常组比较,高血压各组扭转角度明显增大,差异均有统计学意义(P<0.01),高血压各组间比较左室扭转角度差异均有统计学意义(P<0.01)。结论:VVI技术能够简便、无创的评价心脏扭转运动,对高血压引起心脏损害的诊断提供一种有价值的检查手段。 Objective: Apphcatton of w, y o with left ventricular twisting motion, Methods: 20 normal adults and 60 patients with Primary hypertension were Undergone a medical examination by conventional two - dimensional echocardiography, according to the disease progresses, patients with Primary hypertension were divided into three groups: hypertension group (20 cases), hypertension with left ventricular diastolic dysfunction group (20 cases), hypertension with left ventricular hy- dy- 1 pertrophy (18 cases). Under the 3beats condition gatneru g and storeing the two dimensional supersonic namic chart of heart base minor axis section and the heart point minor axis section, the endocardium o[ the pa- tient were outlined under VV| pattern, appling VVI software automatic tracking to analyzed the left room re- verse angle. Resuls:Compari~)n of Ve and SRe compared with the control group, A group only basal septal, an- terior wall and lateral wall of the central early diastolic peak velocity (Ve) decreased, the difference was statisti- cally significant( P〈0.05, P 〈 0.01), the remaining segment of Ve had no significant difference; The most segment of group B (posterior wall of the middle,the inferior wall and apical ventricular seplal except) Ve were significantly lower ( P 〈0.01) ; All segments of C group, Ve were significant lower than the control group(P 〈0.01 ) ;Among three groups of hypertensive patients Ve decreased, the difference between the groups(except for posterior wall of the middle,inferior wall and apical ventricular septal extracellular of the A group and B group) were statistically significant(P 〈 0.05, P〈0.01). Most segments of group A (except of basal lateral wall, the central anterior septal) early diastolic decreasing SRe than those in the control group, the difference was statisti- cally significant ( P 〈 0.05, P 〈 0.01 ) : B group segments (except of central anterior septal) SRe were significant lower than the control group (P 〈 0.01); C group all segments SRe were significant lower than the control group(P 〈 0.01); three groups among patients with hypertension, among all groups (except of A group and B group central anterior septal) the difference was statistically significant(P 〈 0.05, P 〈 0.01). Conclusion: VVI technology is simple, non- wounded evaluation of cardiac reverse the movement, diagnosis of hypertension- induced cardiac damage provides a valuable means of checking.
