乌司他汀对急性肺损伤患者C反应蛋白及基质金属蛋白酶-9的影响 被引量:1

摘要 目的:探讨急性呼吸窘迫综合征(ARDS)患者血C反应蛋白(CRP)和基质金属蛋白酶9(MMP-9)水平的变化及乌司他丁对其的影响。方法:对40例收入ICU内的ARDS患者分为乌司他丁治疗组和对照组,治疗组给予乌司他丁20万U+生理盐水100ml,2次/d静脉注射,其余治疗方案两组均相同,然后对两组患者血CRP和MMP-9水平、呼吸频率、氧合指数(PaO2/FiO2)、PaO2进行测定。结果:两组患者治疗前呼吸频率、PaO2、PaO2/FiO2、年龄及CRP、MMP-9水平均无显著性差异;乌司他丁治疗可显著减少ARDS患者血CRP、MMP-9的表达(P<0.01),显著改善ARDS患者的呼吸情况(P<0.001)。结论:乌司他丁可抑制ARDS的CRP、MMPs表达和改善呼吸情况,对ARDS患者有保护作用。 Objective: To investigate the changes of the blood c-reactive protein (CRP) and matrix metalloproteinase-9 (MMP-9) levels in patients with acute respiratory distress syndrome (ARDS) and the effect of ulinastatin on them. Methods: 40 ADRS patients admitted into ICU were divided into the ulinastatin treatment group and the control group. Patients in the ulinastatin treatment group were intravenously injected with 2× 10^6 U ulinastatin +100 ml 0. 9% normal saline (NS), twice a day. The other treatment plan for both group was the same. Then The levels of blood (CRP) and MMP-9, respiratory rate, oxygenation index (PaO/FiO2), and PaO2 in both groups were determined. Results: There were no significant differences in the respiratory rate, oxygena- tion PaO2 ,PaO/FiO2, ages and the levels of CRP and MMP-9 in patients of both groups before the treatment. Ulinastatin treatment could reduce the expressions of CRP and MMP-9 in patients with ARDS (P〈0.01), and greatly improve the patients' respiration (P〈0. 001). Conclusion: Ulinas- tatin can inhibit the So ulinastatin has a expressions of CRP and MMP-9 and improve the respiration of ARDS patients. protective effect on patients with ARDS.
