新生儿缺氧缺血性脑病患者血浆GMP-140水平变化及意义 被引量:2

摘要 目的 :研究新生儿缺氧缺血性脑病 (hypoxic- ischem ic encephalopathy ,HIE)患者血浆α颗粒膜蛋白 -140 (granule m em brane protein- 140 ,GMP- 140 )水平变化 ,探讨其临床意义。方法 :43例 HIE患儿分为轻、中、重 3组 ,并以正常新生儿作为对照。血浆 GMP- 140采用 EL ISA双抗体夹心法测定。结果 :起病初期 ,血浆 GMP- 140水平显著升高 ,并与病情严重度一致。血浆 GMP- 140水平与血小板数量呈负相关 (r =— 0 .44 ,P <0 .0 5 ) ,另轻度 HIE患儿在血小板数量还没有明显下降时 ,GMP- 140就已较对照组显著升高 (P <0 .0 5 ) ,危重儿在恢复期虽然血小板数量明显上升 ,其 GMP- 140水平仍高于对照组 (P <0 .0 5 )。结论 :HIE患儿体内存在血小板活化 ,血浆GMP- 140的变化与病情的严重度一致。监测患儿血浆 GMP- 140的变化 ,有助于血栓前状态的早期诊断及指导治疗 ,并可作为判断预后的指标。 Objective:To study the changes of plasma GMP 140 (granule membrane protein 140) ,platelet count in newborn infants with hypoxic ischemic encephalopathy(HIE) and to evaluate their clinical significances. Method:43 HIE patients were divided into three groups: mild, moderate and severe. The control group consisted of 19 healthy newborn infant. The levels of GMP 140 was measured by the method of double antibodies ELISA, platelet count was measured by the Coutler stks autoanalysis.Result:The results showed that the plasma level of GMP 140 in HIE was significantly higher than that of control groups(P< 0.01 ). The level of GMP 140 corresponded to the severity of HIE. There was a significant negative correlation between GMP 140 and platelet count (r=- 0.44 ,P< 0.05 ), In addition ,the results also showed that in mild group when platelet count was not decreased significantly ,the plasma levels of GMP 140 had been significantly higher than those in control.When the platelet count increased during the recovery, in the severe group of HIE the GMP 140 were also higher than that of control(P< 0.05 ). Conclusion:These findings indicate that there are elevated platelet activation and prethrombotic state in HIE, the index of GMP 140 is sensitive. The levels of the molecular marker corresponded to the severity of HIE. It is helpful to diagnose prethrombotic state in newborn infants with HIE by examining the level of plasma GMP 140, it also can be considered as prognosis predictors.
