摘要 目的:探讨神经电生理在外伤性臂丛神经损伤诊断中的临床应用价值。方法:应用丹迪KEYPOINT肌电/诱发电位仪对43例外伤性臂丛神经损伤患者进行神经传导速度、诱发定位检查。结果:全臂丛神经损害6例,其中5例正中神经、尺神经、桡神经、肌皮神经、腋神经MCV及SCV均未引出波形,F波消失,患肢体感诱发电位引不出;1例正中神经、尺神经、桡神经、肌皮神经、腋神经MCV明显减慢,波幅明显降低,潜伏期延长,SCV及F波异常。臂丛上干损伤18例,其中肌皮神经、腋神经MCV减慢,波幅下降14例;桡神经、腋神经、肌皮神经MCV减慢,波幅降低4例。臂丛下干损害19例,正中神经、尺神经MCV明显减慢,波幅明显降低,SCV减慢,F波异常者15例;正中神经、尺神经、桡神经MCV减慢4例。结论:神经电生理检查对于正确判断臂丛神经损伤的部位、程度以及决定治疗方案有着重要的临床意义,神经电生理可作为外伤性臂丛神经损伤首选检查。 Objective :To explore the neural electrophysiology in traumatic brachial plexus injury and its clinical value. Methods: Nerve conduction velocity, induced positioning check for 43 cases of traumatic brachial plexus injury patients by Dundee EYPOINT EMG / evoked potential instrument. Results : Six cases brachial plexus damage, including five cases of the median nerve,ulnar nerve, radial nerve,musculocutaneous nerve, axillary nerve MCV and SCV at the end leads wave- form, F wave disappeared, suffering from the limb flu evoked potentials was absent;1 Examplemedian nerve, ulnar nerve, radial nerve, musculocutaneous nerve, axillary nerve MCV slowed down, volatility is significantly reduced latency, the SCV and F-wave abnormalities. Upper trunk of brachial plexus injury in 18 cases, musculocutaneous nerve, axillary nerve MCV slow down ,volatility dropped 14 cases ;radial nerve, axillary nerve, musculocutaneous nerve MCV slow down,volatility low- er four cases. Lower trunk of brachial plexus damage to the 19 cases, the median nerve, ulnar nerve MCV slowed down, volatility is significantly reduced,SCV slow down the F-wave abnormalities in 15 patients ; median nerve, ulnar nerve, ra- dial nerve MCV slow down the four cases. Conclusions:The neurophysiological examination has clinical significance to determine treatment options for brachial plexus injury patients, the neurophysiological can be first choice for traumatic bra- chial plexus injury patients.
