摘要
目的 :分析子宫M櫣ｌｌｅｒｉａｎ腺肉瘤的临床病理特点及其鉴别诊断?椒?:观察 13例子宫腺肉瘤临床病理特征 ,并进行组织化学和免疫组化染色分析。结果 :病人年龄 2 6～ 6 7岁 ,主要表现为阴道不规则流血和子宫增大。肿瘤位于宫体内膜、宫颈内膜和子宫肌壁。镜下腺上皮呈M櫣ｌｌｅｒｉａｎ上皮系列?渲氏赴煨头旨段瘛蚣?,核分裂 4～ 32个 / 10HPF ,具有腺周套袖和息肉样突入腺腔的结构。 4例伴肉瘤过度生长。间质伴横纹肌样 (rhabdoid)分化 2例、伴性索样分化 1例、伴横纹肌肉瘤3例、伴灶性软骨成分 1例。结论 :该肿瘤具有一系列临床病理特征 ,少数可伴肉瘤过度生长 ,间质具有多向分化潜能 。
Purpose To study the the clinicopathologic features and differential diagnosis of müllerian adenosarcoma of the uterus. Methods The clinicopathologic features of 13 cases of adenosarcoma of the uterus were studied and histochemical and immunohistochemical analyses were engaged. Results The patients ranged in age from 26 to 67. The majority presented with abnormal vaginal bleeding and an enlarged uterus. Tumors were located in endometrium, endocervix or myometrium. The glands were lined by a variety of benign müllerian epithelia. The stromal cells showed mild to moderate cytologic atypia, 4 to 32 mitotic figures per 10 high power fields, and characteristic structures of periglangular cuffs of increased cellularity and intraglandular polypoid projections. 4 cases were diagnosed as adenosarcoma with sarcomatous overgrowth. Some areas of the stroma showed rhabdoid element(in 2), sex cord like element(in 1), rhabdomyosarcomatous element(in 3), and focal cartilage(in 1). Conclusion This tumor is a rare one with unique clinicopathologic features, which can help to differentiate them from other tumors. The minority exhibit adenosarcomas with sarcomatous overgrowth. The stroma cells of adenosarcomas have multiplicate differentiation potentiality.
