摘要
目的 观察舌鳞状细胞癌基底膜的Ⅳ型胶原表达及超微结构改变。方法 46例舌癌标本采用免疫组化方法染色观察Ⅳ型胶原的表达 ,其中 3例标本采用透射电镜观察。结果 在肿瘤周围的正常粘膜内 ,Ⅳ型胶原阳性表达且呈连续的线状分布。在癌组织中 ,分化差及伴有淋巴结转移的病例 ,Ⅳ型胶原表达弱甚至无表达 ,但在某些分化较好的部位 ,可发现其表达增强。超微结构观察结果显示 :在胶原纤维完整 ,上皮下结缔组织内成纤维细胞丰富、功能活跃部位 ,基底膜较完整 ,相应部位之上皮细胞分化较好。相反 ,上皮下结缔组织内胶原纤维溶解破坏 ,基底膜破损部位以及成纤维细胞稀少部位 ,上皮细胞分化较低。结论 ①Ⅳ型胶原表达反映了恶性细胞侵袭破坏作用与结缔组织防御修复功能相互作用的结果。②改变上皮下结缔组织健康状况及抗病能力可能是限制舌癌发展的有效措施。
Objective To observe the collagen Ⅳ expression and ultrastructural changes in the basement membrane of oral squamous cell carcinoma. Methods Samples from 46 cases of oral squamous cell carcinoma were stained with immunohistochemical technique for the collagen Ⅳ expression and 3 cases of the patients were observed by the transmission electron microscope. Results Positive expression and continuous linear distribution of collagen Ⅳ expression were observed in normal mucosa near the tumors, while weak even missing expression were found in the carcinoma tissues with poor differentiation and lymphatic metastasis. Ultrastructurally, where the collagenous fibrils were intact and fibroblasts with lively functional action were found in the underlying connective tissues, the basal lamina were intact, the epithelial cells were well differentiated. On the contrary, where the collagenous fibrils dissolved and damaged, the basement membrane damaged and the fibroblasts rarely found in the connective tissues, the epithelial cells were poorly differentiated. Conclusion ①Collagen Ⅳ expression is the interacting results of the invasion and damage function of the malignant cells and the defense and repairing function of the connective tissue. ②Keeping a healthy condition for the hypodermal connective tissue may be a valuable method to restrain the oral squamous cell carcinoma development.
