低温保护技术在腹腔镜肾部分切除术中的应用进展 被引量:16

摘要 近20年来,随着影像检查技术如超声、CT、MRI等的不断发展,小肾癌(〈4elTl)的检出率明显增加。对于T,期肾癌,2012年欧洲泌尿外科协会推荐的治疗方案为保留肾单位的肾部分切除术。在肾部分切除术中,为保持视野清晰以及控制术中出血、缩短手术时间,往往需要阻断肾门血管[1]。阻断肾血管可引起肾脏缺血及再灌注损伤,如何在阻断肾血管的同时,减少因血流中断引起的肾缺血损伤是当前研究的热点。肾脏的低温保护是指通过各种低温保护材料,降低肾脏温度,从而减轻肾缺血及再灌注损伤,延长缺血耐受时间。本文简述肾缺血再灌注损伤的机制以及对肾缺血耐受时间的最新认识,分析3种不同肾脏低温保护方法的优缺点,以促进腹腔镜肾部分切除术的发展。
