摘要
目的 探讨在腹腔镜下不阻断肾蒂对小体积的肾肿瘤行肾部分切除术的可行性和临床效果.方法收集2008年9月～2013年9月20例肾占位病例,采用不阻断肾蒂的小体积肾部分切除术.肿瘤直径2.2～3.5cm,平均2.95cm.结果 20例患者均成功手术,手术时间45～70min,平均时间65min;术后病理报告提示切缘均为阴性;术中出血30～100mL,平均75mL;术后平均住院时间为7.2d;20例患者随访至今,无一例出现肿瘤局部转移或复发.结论 不阻断肾蒂的肾部分切除术安全有效,可以减少肾脏热缺血时间,并有利于患者的术后恢复.
Objective To investigate the feasibility and clinical result of laparoscopic partial nephrectomy without blocking renal pedicle on a small volume of renal tumors. Methods From September 2008 to September 2013, 20 cases of patients with renal masses were used with partial nephrectomy without blocking renal pedicle. Tumor diameter 2.2- 3.5cm, average 2.95cm. Results Twenty patients were successfully operated. The time were 45-70min, the average time 65min; pathology reports suggested that margins were negative; bleeding 30-100mL, average 75mL; mean postoperative hospital stay was 7.2 days; 20 cases patients were followed up to now, and there was no case of local tumor metastasis or recurrence. Conclusion No-clamping renal pedicle partial nephrectomy is safe and effective, which can reduce renal warm ischemia time, and enable the patients＇ postoperative recovery.
