摘要
目的调查医院住院药房退药情况,分析退药原因,采取有效的防范措施,促进医院合理用药,提高药房管理水平。方法收集我院住院药房5 962例退药申请单,从退药科室、退药原因、种类、金额进行统计分析并提出建议。结果总退药比例为10.55%,退药量最大的科室为儿科,所占比例也最大,为17.15%,存在退药的药品比例最大的是抗菌药物,主要退药原因有出院、变更医嘱、处方错误等。结论强化医生合理用药意识,加强药品管理,进一步完善信息系统,有效控制退药,保证临床用药安全。
Objective To investigate the current situation of drug withdrawal in our in-hospital pharmacy, and to analyze the causes, in order to provide the effective arrangement for improving the rational drug use and hospital management level. Methods Statisti- cal analysis on the 5 962 cases application of the drug withdrawal in our in-hospital pharmacy was carried out,and rational sugges- tions was put forward. Results Total drug withdrawal proportion was 10.55 % ,among which department of pediatrics occupied the largest amount（17.15 % in total）. The main category of accidents of drug withdrawal were as follows：patients were discharged from hospital,doctors changed orders, the prescription mistook, and so on. Conclusion We should strengthen the drug manage- ment, improve the sense of responsibility and professional qualities, and focus on perfecting information system, in order to guaran- tee the drug use safety.
