国内首次从泛耐药铜绿假单胞菌中检出KPC型β-内酰胺酶基因 被引量:2

摘要 目的调查一组泛耐药铜绿假单胞菌中β-内酰胺酶基因和膜孔蛋白基因的存在情况。方法收集2010年1-12月浙江绍兴第二医院住院患者痰液标本中分离到的泛耐药铜绿假单胞菌共20株,用聚合酶链反应(PCR)及序列分析的方法分析15种A类β-内酰胺酶基因、10种B类β-内酰胺酶基因、2种C类β-内酰胺酶基因、4种D类β-内酰胺酶基因检测以及膜孔蛋白oprD2基因。结果 20株泛耐药铜绿假单胞菌β-内酰胺酶基因检出率,TEM为90.0%、CARB为100.0%、KPC为5.0%(经测序和BLASTn比对,确认为KPC-2型)、OXA-10群为10.0%,膜孔蛋白oprD2基因均为缺失。结论 20株泛耐药铜绿假单胞菌均存在产β-内酰胺酶和膜孔蛋白缺失是对β-内酰胺类药物耐药的重要原因;从泛耐药铜绿假单胞菌中查出KPC型β-内酰胺酶基因是国内外首次报道。 OBJECTIVE To investigate the distribution of β-1actamase genes, and outer membrane porin gene in a group of pandrug-resistant Pseudomonas aeruginosa. METHODS From Jan 2010 to Dec 2010, 20 strains of pandrug-resistant P. aeruginosa were collected from sputum samples of inpatients in Shaoxing Second Hospital, Zhejiang Province, China. Then, 15 kinds of class A β-1actamase genes, 10 kinds of classB β-lactamase genes, 2 kinds of class C β-lactamase genes, 4 kinds of class D β-1actamase genes, and outer membrane porin gene oprDz were analyzed by PCR and DNA sequencing. RESULTS In 20 strains of pandrug-resistant P. aeruginosa, 4 kinds of β-1actamase genes were positive: 18 strains of TEM (90.00% ), 10 strains of CARB (100.0 % ), 1 strain of KPC- 2 (5. 0%), 2 strains of OXA-10 group (10. 0%). All outer membrane porin gene oprD2 was negative. CONCLUSION In this group of pandrug-resistant P. aeruginosa, producing β-1actamase genes and loss of oprD2gene play a key role in the resistance to β-1actams. And it's the first report that KPC are discovered in P. aeruginosa in China.
