摘要
目的:研究骶神经根对盆底器官逼出肌和括约肌的功能支配,为临床从骶神经根角度重建脊髓损伤(SCI)后的排泄功能提供依据。方法:对T10截瘫犬进行硬膜外L7-S3神经根电刺激,16只犬同时记录膀胱和尿道压力变化;7只犬同时记录直肠和肛门压力变化。结果:犬道尿肌的神经支配以S2为主,S1次之;S2单独提供了膀胱压力增高值的45.2%,S1+S2提供了80.7%。尿道括约肌的神经支配以S1为主,S2次之;S1单独提供了尿道压力增高值的43.4%,S1+S2提供了76.7%。犬直肠的运动支配以S2为主,S1、S3次之;肛门的运动支配以马为主,S1、S2次之。对盆底器官的运动神经支配效能,均存在着右侧骶神经根大于左侧的特点。结论:盆底器官的逼出肌与括约肌具有不同的主要支配神经根。这种功能支配的侧重和交叉,对SCI后排泄功能的重建有一定意义。
To provide experimental basis for neural reconstruction of micturition and defecation in SCI patients. Methods: After the transection of T10 and laminectomy of L4 - L7, bilateral L7 - S3 roots were electrostimulated separately in 16 mongrel dogs. The pressure of urocyst and urethra were recorded synchronously in all those 16 dogs, while the pressure of rectum and anus were recorded in 7 dogs. Results; S2 is the most dominant nerve of urinary detrusor. 45.2% of total pressure increase is attribute to S2 while 80.7% is attribute to S1 and S2 S1 is the most dominant nerve of urinary sphincter. 43 .4% of total pressure increase is attribute to S1, while 76.7% is attribute to S1 and S2.For tectal destusor, S2 is the most dominant nerve, with S1 and S3 at a less degree.For anal sphincter, S3 is the most dominant nerve, with S1 and S2 at a less degree.For both lower urinary and gastroenterology, the right sacral roots provide more innervation than the left ones. Conclusion: It is important for the reconstruction of micturition and defecation in SCI patients that the detrusors and sphincters of pelvic organ are innervated by different dominant nerve in spinal roots.
