摘要
目的建立一套规范完整的预防下肢深静脉血栓（DVT）的老年围手术期护理安全管理流程，指导老年围手术期临床护理。方法通过对70例老年骨科围手术期患者DVT危险因素的分析，量化DVT风险评估系统；依据风险级别实施对应的预防护理措施，监测患者腿围、血D·Dimmer及血管超声评价围手术期护理疗效。结果70例患者均接受了预防深静脉血栓护理措施并量化监测护理疗效，围手术期无一例发生DVT；59例患者依从性良好，良好率为84．29％；62例患者舒适度满意，满意率为88．57％；70例患者术后14d血D．Dimmer检查均在正常范围；术后3d患者下肢腿围膝上10em处及膝下10cm处分别为（49．910±1．556），（37．320±1．803）cm，与术前的（49．170±1．535），（37．100±1．823）em比较，差异均无统计学意义（P〉0．05）。结论建立一套规范完整的老年围手术期预防DVT的护理安全管理流程，并用于指导临床护理工作，能有效预防老年围手术期DVT的发生。
Objective To establish standard and complete nursing safety management process to prevent peri-operative deep venous thrombosis （DVT） in elderly orthopedics patients and guide the clinical nursing. Methods The risky factors of DVT in 70 peri-operative elderly orthopedics patients were analyzed, and risk evaluation system was formulated. Corresponding measures were taken according to the level of risks, and muscle circumference, D-Dimmer and vascular ultrasound was measured to evaluate nursing effect. Results All 70 patients received nursing measures to prevent DVT and the effect was monitored, and there was no DVT during peri - operative period. 50 patients had better compliance, with the rate of 84.29% , 62 patients were satisfied with the comfort status, with the rate of 88.57%, and all 70 patients had normal D-Dimmer examination results 14 days after operation. Patients＇ muscle circumferences 10 cm above and below the knees were respectively （49. 170 ± 1. 535） cm and （37. 100± 1. 823 ） em before operation, and （49. 910 ±1. 556 ） cm and （37. 320 ± 1. 803） cm 3 days after operation, and there was no statistically significant difference （P 〉 0.05 ）. Conclusions Establishment and clinical application of standard and complete nursing safety management process can effectively prevent peri-operative deep venous thrombosis （DVT in elderly orthonedics natients.
