摘要
目的 :观察 EB病毒感染与癌基因、抑癌基因在鼻咽癌 ( NPC)中的表达与相互关系 ,研究 NPC发生的有关机制。方法 :免疫组化检测 5 0例 NPC组织中 EB病毒潜伏膜蛋白 ( L MP1 )癌基因蛋白、细胞周期素 D1 ( Cyclin D1 )、H-ras、c-fos,抑癌基因蛋白P16、RB的表达。结果 :NPC中 L MP1 、Cyclin D1 、H-ras、c-fos、P16、RB阳性检出率分别为 64 %、5 6%、68%、60 %、3 8%、72 %。L MP1表达在 NPC WHO 型中显著高于 WHO 型 ( P<0 .0 1) ,在转移组中表达亦增加。Cyclin D1 在癌呈巢性膨胀性生长中显著高于其他类型 ( P<0 .0 1) ,RB表达在 WHO 型中显著高于 WHO 型 ( P<0 .0 5 ) ,P16在转移 NPC中阳性表达显著低于无转移组 ( P<0 .0 1) ,LMP1 表达与癌基因 Cyclin D1 、H-ras、c-fos阳性表达密切相关 ( P<0 .0 5 )。结论 :在 NPC中有较强的 L MP1 阳性表达 ,说明肿瘤细胞中 EB病毒以潜伏感染为主 ,常伴有细胞表型的改变 ,NPC发生是多步骤、多基因改变的过程。
Objective:To investigate the expression of oncogenes including Cyclin D 1,c-fos,H-ras and suppressor genes including P16,Rb,in NPC,in relation to the EB Virus infection,or both.Methods:Formalin fixed,Paraffin embedded tissue from 50 NPC was examined by immunohistochemistry for Latent Membrane Protein(LMP 1),Cyclin D 1,c-fos,H-ras,P16,Rb.Results:The protein expressions of LMP 1,cyclin D 1,H-ras,c-fos,P16 and RB were detected in 64%,56%,68%,60%,38% and 72%,respectively.Tumor with LMP 1 aberrant,expression was significantly higher in the nonkiratinzing NPC (WHO Ⅱ type)and metastatic group than that in the undifferentiated NPC (WHO Ⅲ type) and nonmetastatic group. (P<0.05). Cyclin D 1 expression was significantly higher in the Regaud growth pattern than that in the other growth patterns (P<0.01). RB expression was significantly higher in the WHO Ⅱ type of NPC than that in the WHO Ⅲ ones. P16 showed lower expression in the metastatic group than that in the nonmetastatic group (P<0.05). A significant correlation was found between LMP 1 ,positive expression and oncogenes,cyclin D 1,H-ras,c-fos (P<0.05).Conclusion:The stronger expression of LMP 1 in NPC indicates that the virus is latent infection in the tumor cells and accompanied by changes in the cellular phenotype.The tumorigenesis of NPC may be thought to be a multistep process and involved multiple genetic changes.
