摘要
目的分析初诊远处转移鼻咽癌患者的临床病理特征,探讨鼻咽癌远处转移的临床相关因素。方法对42例初诊远处转移鼻咽癌患者的各项临床病理特征包括性别、病程、首发症状、T分期、N分期、颈淋巴结转移位置(侧数,单侧/双侧)及病理类型及进行分析。结果全部患者中,不同病程、首发症状、T分期、N分期及病理类型等五项特征的病例分布均匀,差异无统计学意义,X2值分别为7.714、5.339、0.0950、.381、2.381,P值分别为0.0520、.149、0.758、0.537、0.123。不同性别及颈淋巴结转移位置(侧数,单侧/双侧)两项特征的患者分布不均匀,差异有统计学意义,X2值分别为6.0955、.158,P值分别为0.014、0.023。结论鼻咽癌初诊时远处转移的发生与病程、首发症状、T分期、N分期、病理类型等临床病理特征无相关性。男性、双侧颈淋巴结转移的鼻咽癌患者初诊时更多见远处转移,对此类患者应采取积极的综合治疗措施。
Objective To investigate the factors related to distant metastasis of nasopharyngeal carcinoma through analyzing the clinical and histopathological characters of nasopharyngeal carcinoma patients with distant metastasis at preliminary diagnosis.Methods Several clinical and histopathological characters were analyzed for 42 patients with nasopharyngeal carcinoma with distant metastasis at preliminary diagnosis.These factors included sex,course of disease,symptoms seen in the primary complain,T stage,N stage,location of neck masses（unilateral or bilateral neck node metastasis） and histopathological types.Results There was no difference in the distribution of cases with the factors including course of disease,symptoms seen in the primary complain,T stage,N stage and histopathological types.X^2=7.714、5.339、 0.095、0.381、2.381 respectively,P=0.052、0.149、0.758、0.537、0.123 respectively.There was significant difference in the distribution of cases with the factors including sex and location of neck masses（unilateral or bilateral neck node metastasis）.X^2=6.095、5.518 respectively,P=0.014、0.023 respectively.Conclusion There was no relativity between the distant metastasis and the most clinical characters.There was the more risk of distant metastasis at preliminary diagnosis in the nasopharyngeal carcinoma patients who is male or bilateral neck node metastasis.Active synthetical therapy should be applicable for such cases.
