摘要
目的 探讨螺旋CT增强扫描反映肝细胞癌雌激素受体 (ER)表达的可能性。方法 经病理证实的肝细胞癌患者 43例 ,其中动态扫描 2 6例 ,评价时间 -密度曲线走势并计算癌灶强化峰值(PV)和强化比值 (CER) ;动、门脉双期扫描 17例 ;观察癌灶强化表现特征、包膜、子灶和门脉癌栓。组织切片经ER免疫组化染色。结果 癌细胞ER阳性 2 1例 ,阳性率 48.8% ,ER阳性组肿瘤平均横径5 .1cm ,ER阴性组为 7.0cm。ER阳性组癌灶PV、CER小于ER阴性组。有明确包膜组ER阳性率高 ,伴发子灶、门脉癌栓组ER阳性率低。
Objective To investigate the enhancement imaging features of hepatocellular carcinoma (HCC) and relevant parameters revealed by spiral CT scanning, and its correlation with the expression of estrogen receptor (ER). Methods The study included 43 histopathologically proven HCC patients. Among them, 26 cases underwent single level dynamic spiral CT scanning and the portal venous phase scanning of whole liver, and the rest 17 cases underwent dual phase scanning of whole liver. We evaluated the time density curve (T DC) and calculated the peak value (PV), and contrast enhancement ratio (CER). The enhancement imaging features, pseudocapsules, intrahepatic daughter foci, and tumor emboli in portal veins were observed. Slides were stained with the standard immunoperoxidase method using the estrogen receptor (ER) monoclonal antibody to determine the ER expression. Results Twenty one cases were ER positive (48.8%). The mean diameter of the ER positive lesions were 5.1 cm, while the ER negative cases were 7.0 cm. The PV and CER of HCC lesions in the positive cases were lower than that in the negative cases. ER positive percentage was higher in cases with pseudocapsules than in that without. ER positive percentage was very low in cases of HCC lesions with intrahepatic daughter foci and tumor emboli in portal veins. Conclusion The enhancement imaging features of HCC revealed by spiral CT could indicate the expression of ER to a certain degree.
