胱抑素C和同型半胱氨酸与糖尿病肾病的相关性研究 被引量:21

摘要 目的探讨2型糖尿病(T2DM)患者血清胱抑素c(CysC)和同型半胱氨酸(Hcy)水平对于诊断早期糖尿病肾损伤的临床意义。方法比较单纯糖尿病组(SDM)、早期糖尿病肾病组(EDN)、临床糖尿病肾病组(CDN)与对照组(NGT)患者间血清CysC、Hcy水平和24小时尿白蛋白排泄率(UAER),应用MDRD简化公式计算肾小球滤过率(GFR)并进行分析。结果2型糖尿病患者EDN组及CDN组的Hcy、CysC水平均高于SDM组及NGT组,差异有统计学意义(P〈0.05),CysC、Hcy分别与UAER及血肌酐(Scr)呈正相关,与GFR呈负相关。结论CysC和Hcy是能够反映早期糖尿病肾损伤的标志物。 Objective- To investigate the association of Cystatin C (Cys C) and Homocysteine (Hcy) with early diabetic kidney injury in type 2 diabetes mellitus(T2DM) patients. Methods Accord- ing to the level of urinary albumin excretion rate(UAER) ,patiants of T2DM were divided into 3 groups, single DM group(SDM,UAER 〈30 mg/24 h) ;early stage of diabetes nephropathy(EDN,30 mg/24 h 〈 UAER 〈300 mg/24 h);Clinical stage of diabetes nephropathy( CDN,UAER 〉300 mg/24 h). Compara- tive analysis of Cys C, Hey and UAER were made in these groups. At the same time using MDRD simpli- fied formula calculating glomerular filtration rate(GFR) for comparative analysis. Results The levels of Cys C, Hey in EDN and CDN groups were significantly higher than those in SDM group and NGT group ( P 〈 0.05 ). Cys C and Hey were correlated to UAER and Scr. Hey and Cys C were negatively correlated to GFR. Conclusion Cys C and Hey might be markers to reflect the early diabetic kidney injury.
