摘要
目的观察不同剂量的布托啡诺复合异丙酚用于无痛人流的麻醉效果及安全性。方法选择ASA I~Ⅱ级拟行门诊宫腔镜手术患者150例,随机分为5组:P组(0μg/kg)、B5组(5μg/kg)、B10组(10μg/kg)、B15组(15μg/kg)、B20组(20μg/kg)。各组静脉注射布托啡诺后缓慢推注异丙酚2mg/kg,必要时追加异丙酚;记录患者意识消失时间、手术时间、苏醒时间及出院时间;血流动力学变化和异丙酚用药量;麻醉效果分级、下腹痛视觉模拟评分(VAS)及不良反应等。结果与P组相比,B5~B20组麻醉效果更好、下腹痛VAS评分更低、苏醒时间明显缩短、异丙酚总用量显著降低(P<0.05)。B10~B20组较B5组镇痛效果更完善、异丙酚总用量明显减少(P<0.05),而B10~B20组镇痛效果VAS评分之间差异无统计学意义(P>0.05)。B15~B20组呼吸抑制的发生率较B5~B10组明显增高(P<0.05)。结论 10μg/kg布托啡诺复合异丙酚用于无痛人流手术麻醉效果,镇痛效果好、不良反应发生率低,更适于门诊手术。
Objective To investigate the effects and safety of butorphanol and propofol in operation of painless induced a- bortion. Methods 150 ASA Ⅰ-Ⅱ patients undergoing outpatient painless induced abortion were randomly divided into five groups according to different dose of butorphanol. Followed by intravenous injection of propofol 2mg/kg, the patients were induced with butorphanol 0,5,10, 15,20μg/kg in group P, B5, B 10, B 15, B 20 respectively. Anesthesia was maintained with propofol when needed. The following were recorded and compared in each group : the time of unconsciousness, operation time, recovery time from anesthesia, discharge time; hemodynamic changes during anesthesia and the total doses of propofol ; anesthesia satisfaction rating, VAS of pain and the side effects. Results Anesthesia satisfaction rating, recovery time from anesthesia and the total doses of propofol were significantly improved in group B 5, B 10, B 15, B 20 ( P 〈 0. 05 ). Conclusion with group B5, Anesthesia satisfaction rat- ing was better and the total doses of propofol was less in group B10, B15, B20 ( P 〈 0. 05 ). However, there were no significant difference among group B10, B15 and B20 regarding VAS of pain (P 〉0. 05). The incidence of respiratory depression was signifi- cantly higher in group B 15 and B20 compared with group B 5 and B 10 (P 〈 0. 05 ). Conclusion 10μg/kg butorphanol combined with propofol anesthesia appears to be superior for painless induced abortion, with regards to better anesthetic effects and less adverse effects.
