摘要
目的探讨老年患者Barrett食管(Barrett's esophagus,BE)的临床及内镜表现与病理特点。方法将748例经病理确诊的BE患者根据年龄划分老年组(n=156)和非老年组(n=592),并对其临床及内镜表现与病理特征进行对比分析。结果老年组典型反流症状(17.4%)所占比例明显高于非老年组(9.0%),且两组间差异有统计学意义(P<0.05);两组患者胃镜下表现均以短段BE及岛状BE为主,其中老年组全周型BE(19.2%)所占比例高于非老年组(8.3%),两组间差异有统计学意义(P<0.05);老年组肠上皮化生(23.7%)所占比例高于非老年组(13.8%),两组间差异有统计学意义(P<0.05);非老年组检出异型增生3例,有杯状细胞2例。结论老年人易出现典型的反流症状,内镜下全周型BE较非老年人多,老年人BE肠上皮化生的发生高于非老年人,国人中存在杯状细胞的BE较罕见。
Objective To explore clinical and endoscopic manifestations and pathological characteristics in the elderly patients with barrett esophagus（ BE）. Methods We collected 748 BE patients which confirmed by pathologic examination into analysis. And the cases were divided into elderly group （n = 156） and non - elderly group （n = 592） according to age. The clinical features, endoscopic findings and pathological characteristics were comparative analyzed. Results The proportion of typical reflux symptoms of elderly group （ 17.4% ） was significantly higher than non - elderly group （9.00） （P 〈 0.05 ）. The main gastroscope performance were short - segment BE and islands pattern BE in two groups, in which the proportion of ring pattern in elderly group （19.2%） was significantly higher than the non - elderly group （ 8.3 % ）, and the difference between the two groups had statistical significance （ P 〈 0.05 ）. The detection rate of intestinal metaplasia in the elderly group （23.7 % ） was significantly higher than in the non - elderly group （ 13.8 % ） （ P 〈 0. 05 ）. There were 3 ca- ses of dysplasia （mild dysplasia） in the non - elderly group. And the Goblet cells were only detected in 2 cases. Conclusion The elderly prone to more probability of typical reflux symptoms than non - elderly. The detection rate of ring pattern was significantly higher in the elderly than non - elderly. The detection rate of specific intestinal metaplasia in the elderly was significantly higher than in the non - eld- erly. The Goblet cells was relatively rare in Chinese BE patients.
