上皮性卵巢癌患者外周血中RRM2的表达及临床意义 被引量:1

摘要 目的:探讨核糖核苷酸还原酶M2(RRM2)在上皮性卵巢癌患者外周血中的表达,初步探讨其作为肿瘤标记物在上皮性卵巢癌诊断中的意义。方法:应用实时荧光定量逆转录聚合酶链反应(real-time RT-PCR)检测70例上皮性卵巢癌、16例低度潜能卵巢恶性肿瘤、15例卵巢良性肿瘤患者和15例健康外周血标本单个核细胞中的RRM2相对表达量。结果:(1)上皮性卵巢癌患者与低度潜能恶性肿瘤患者外周血中的RRM2 mRNA相对表达量显著高于良性卵巢肿瘤患者和正常对照组(P<0.05),后两组比较则无显著差异(P>0.05);(2)RRM2 mR-NA表达随临床分期的进展呈上升趋势(P<0.05),RRM2 mRNA表达量与FIGO分期、腹水、组织分化、淋巴转移及大网膜转移有关(P<0.05),与患者年龄、组织学分型无关(P>0.05);(3)上皮性卵巢癌患者外周血中RRM2 mRNA与血清中CA125、HE4的表达呈正相关(P<0.05);(4)ROC曲线分析显示,RRM2 mRNA临界值为1.759时诊断指数最大(敏感性94.3%,特异性76.6%),AUC=0.896(95%CI 0.819~0.948,P=0.000)。结论:RRM2 mRNA可能在上皮性卵巢癌的发生发展中发挥癌基因的作用,有望成为诊断上皮性卵巢癌的新指标。 Objective:To investigate the detection of ribonucleotide reductase M2 (RRM2) in epithelial ovarian cancer samples and the potential of using RRM2 in peripheral blood as biomarker for epithelial ovarian cancer. Method:The expressions of RRM2 in peripheral blood from 70 cases of epithelial ovarian cancer and 16 cases of borderline ovarian neoplasm and 15 cases of benign epithelial ovarian tumor and 15 eases of normal controls were detected by real-time RT-PCR. Resuit:( 1 )The expressions of RRM2 mRNA in epithelial ovarian cancer and borderline ovarian neo- plasm were significantly higher than those in benign epithelial ovarian tumor and normal subjects(P 〈0.05 ). There was no significant difference between the two latter groups. (2)The expression of RRM2 mRNA increased with the progression of the clinical stage ( P 〈0.05 ). The expression of RRM2 mRNA in epithelial ovarian cancer was relevant to FIGO stage ,presence of ascites,pathologi- cal differentiation ,lymph node metastasis and omental metastasis (P〈0.05) ,but it had no correlation with age,histological types (P〉0.05). (3)Expression of RRM2 mRNA was positively correlated with CA125 and HF4 in epithelial ovarian cancer serum(P〈0.05). (4)The receiver operating characteristics curve revealed that the serum R1LM2 mRNA level of 1. 759 significant had the roost diagnostic index,with a sensitivity of 94.3% and specificity of 76.6%. The ROC area under the curve was 0. 896 (95% CI 0.819 -0.948, P = 0.1300). Conclusion :RRM2 may play a role of an oncogene in the occurrence and development of ovarian epithelial carcinoma and could serve as a new target of early diagnosis of ovarian cancer.
