摘要
患者男,18岁。全身水疱伴瘙痒20d。皮肤科情况:头皮可见片状的环形红斑,边缘有渗出,呈束状发。躯干及四肢可见散在或密集分布的片状红斑,其上见针尖大水疱,呈环状排列,中央轻度糜烂、坏死及结痂。部分小水疱融合成大疱,尼氏征(-)。背部皮损组织病理:表皮下可见大疱形成,疱内可见浆液性物质及嗜中性粒细胞。直接免疫荧光检查:IgA基底膜带线状沉积。诊断:成人线状IgA大疱性皮病。予氨苯砜治疗1个月,疗效较好。
A 18-year-old man presented with a 20-day-history of blisters involved the whole body with itching. Physical examination： revealed multiple erythema annulare and sarciniform hair on scalp, exudation on border, Ery-thema annulare on the trunk and the four limbs by the circular blister which was needle size, Mild erosion, necrosis and crust were on the centre lesion. Partly blister fused big ones, Nikolsky＇s sign were negative. Histopathologic examination showed a subepidermal blister with serum substance and neutrophils infiltrate. Direct immunofluorescence showed continuous linear deposits of IgA in the basement membrane zone. Diag-nosis： Linear IgA Bulous Dermatosis in adult. After one-month treatment with dapsone, marked improvement was observed.
