摘要
目的为了降低食管癌切除术后颈部食管胃吻合口瘘的发生率 ,对手术进行改进。方法①将常规牵拉胃底把胃提至颈部改为牵拉胃贲门部。②食管胃深浅间隔进针吻合法。③胃体浆肌层切开减张降低吻合口存在的张力。结果连续进行食管癌切除颈部食管胃吻合手术101例 ,发生吻合口瘘1例。结论该手术改进可明显降低颈部食管胃吻合口瘘的发生率。
Objective To modify the procedures of the esophageal operation so as to reduce the occurrence of the esophagogastrostomy fistula after resection for esophageal carcinoma. Methods The fundus of the stomach was pulled to the cervical part of the esophagus and then suture the superficial layer and the deep layer of the esophagus with the stomach alternately. The seromuscular layer was cut so as to reduce the tension at the anastomosis. Results 101 consecutive patients were operated by the modified teduigue with only one fistula. Conclusion The present procedure was effective in reducing the occurrence rate of the esophagogastrostomy fistula after the esophageal carcinoma resection.
