摘要
新农合支付方式改革是控制医疗药品费用不合理增长、保障新农合制度长期稳定运行的重要手段。其本质是以预付制取代后付制,将基本医保的基础性作用发挥至最大化,全面实现医疗机构补偿机制和激励机制的转换。改革应坚持三个"加强原则",即加强基金收支预算管理,实施总额预付原则;加强门诊统筹,实施按人头付费原则;加强大病保障,实施按病种付费原则。
The reform of payment of NRCMS is a very important means to control the medical costs of medicine and safeguard the long-term stable operation of NRCMS. The nature is that payment should be replaced by prospective payment system, give full play to basic function of medical insurance, and realize the transformation from compensation mechanism to incentive mechanism of medical institutions. This reform should adhere to three ＂strengthening the principles＂. First, it is important to strengthen the management of revenue and expenditure and budget of fund and adhere to global budget principle. Secondly, it is essential to strengthen the coordination of outpatient and adhere to paying per capita principle. Finally, it is vital to strengthen the serious illness protection and adhere to the principles of DRGs.
