糖尿病合并高血压患者围术期血浆P物质水平的研究

摘要 目的观察糖尿病合并高血压患者围术期血浆P物质(SP)的变化。方法选择28例糖尿病(DM)患者作为实验组,分为2个亚组:合并高血压的糖尿病亚组(DM-HP)17例和正常血压糖尿病亚组(DM-NBP)11例,同时选28例无糖尿病和高血压患者作为对照组(C组),应用酶联免疫分析方法,测定各组患者术前、术中、术后血浆SP浓度。结果 DM-HP患者血浆SP显著低于C组和DM-NBP组(P<0.001),但各组患者围术期SP变化差异无统计学意义(P>0.05)。结论 DM-HP患者血浆SP明显降低,提示SP的降低可能与糖尿病和高血压病有关,手术应激状态下,SP对机体的生理性保护作用可能被削弱,糖尿病合并高血压患者术中发生重要脏器功能障碍的风险增加。 Objective To investigate the perioperative changes of plasma levels of substance P (SP) in diabetics with and without a history of hypertension (HP). Methods Selected twenty-eight patients with diabetes as experimental group, such patients would be divided into two sub-groups: seventeen diabetics with hypertension (DM-HP) in 17 cases and eleven diabetics with normal blood pressure (DM-NBP) in 11 cases. At the same time, twenty-eight patients without diabetic and hypertension were selected as control group. Plasma levels of SP in preoperative, intraoperative and postoperative were determined using enzyme-linked immunosorbent assay (ELISA). Results SP levels in DM-HP patients were apparently lower than those in the C group and DM-NBP group during perioperative period (P 〈 0.001). However, the levels of SP had no significant difference during perioprative in each group (P 〉 0.05). Conclusion SP levels in DM-HP patients are significantly decreased, which may be related to diabetic and hypertension. In the condition of surgery stress, the physiological protective effect of SP may be weakened, and the risk of vital organs dysfunction will be greatly increased in these patients during perioperative period.
