摘要
目的:探讨术后早期应用肠内高营养治疗对晚期胃癌患者白细胞介素(IL)-2系统水平的影响。方法:98例晚期胃癌术后患者随机分成早期肠内高营养组和常规输液对照组,采用酶联免疫吸附测定(ELISA)等方法检测两组患者治疗前后的外周血清IL-2、IL-2受体(IL-2R)α及可溶性IL-2R(sIL-2R)水平。结果:早期肠内高营养组患者冶疗1周后,机体IL-2和IL-2Rα水平较治疗前显著上升,sIL-2R水平明显下降(治疗前:IL-224.83U/ml±2.10U/ml,IL-2Rα17.8%±2.8%,sIL-2R347.9U/ml±57.8U/ml,治疗后:IL-279.82U/ml±6.88U/ml,IL-2Rα27.5%±4.0%,sIL-2R157.9U/ml±24.3U/ml);而常规输液对照组患者的IL-2水平较治疗前呈下降趋势,IL-2Rα和sIL-2R水平变化不明显(治疗前:IL-221.87U/ml±1.98U/ml,IL-2Rα18.3%±3.8%,sIL-2R479.3U/ml±69.8U/ml;治疗后:IL-211.34U/ml±0.89U/ml,IL-2Rα17.4%±2.7%,sIL-2R481.2U/ml±70.4U/ml),两组治疗后结果相比有显著差异。结论:早期应用肠内高营养治疗晚期胃癌术后患者,有利于尽快恢复患者的机体免疫调节功能,为今后临床开展该项治疗提供了证据。
Background/Aims: To study the effects of early enteral hyperalimentation therapy on interleukin (IL)-2 system of postoperative advanced gastric cancer patients. Methods : 98 postoperative advanced gastric cancer patients were randomly divided into two groups, early enteral hyperalimentation group and routine intravenous infusion group. Serum IL-2, IL-2 receptor (IL-2R) α and soluble IL-2R(sIL-2R) levels in both groups were measured by enzyme-linked immunosorbent assay (ELISA). Results:ln early enteral hyperalimentation group IL-2 and IL-2R7 levels were increased markedly and sIL-2R level was decreased after one-week treatment (before treatment: IL-2 24.83 U/ml ± 2.10 U;/ml, IL-2Rα17.8% ± 2.8%, sIL-2R 347.9 U/ml ± 57.8 U/ml; after treatmentt IL-2 79.82 U/ml± 6.88 U/ml, IL-2Rα 27.5%± 4.0%, sIL-2R 157.9 U/ml±24.3 U/ml), while in the routine intravenous infusion group there was a decrease in IL-2, but no changes in IL-2R7 and sIL-2R levels (before treatment: IL-2 21.87 U/ml ±1.98U/ml, IL-2Rα 18.3% ±3.8%, sIL-2R 479.3 U/ml±69.8 U/ml, after treatmentt IL-2 11.34 U/ml±0.89 U/ml,IL-2Rα17.4% ±2.7%, sIL-2R 481.2 U/ml ±70.4 U/ml). Significant differences were found in IL-2, IL-2Rαand sIL-2R levels between the two groups after one-week treatment. Conclusions: Early enteral hyperalimentation therapy is helpful in restoration of the immuno-modulating function of postoperative advanced gastric cancer patients.
